Position Summary

The Tailored Care Manager Supervisor is responsible for overseeing the daily operations and performance of Tailored Care Managers and related staff within a community-based setting. This role ensures the delivery of high-quality, person-centered care coordination services that support optimal health outcomes. The Supervisor provides leadership, guidance, and support to care management staff ensuring compliance with regulatory standards, and promoting effective collaboration among healthcare providers, community partners, and support networks.

Key Responsibilities

• Supervise, coach, and evaluate Tailored Care Managers, care manager extenders, peer support specialists, and community health workers.
• Provide guidance on complex cases, ensuring appropriate assessment, care planning, and service coordination.
• Oversee the development, implementation, and ongoing review of individualized care plans to ensure quality and compliance.
• Monitor staff productivity, documentation quality, and adherence to organizational and regulatory standards.
• Conduct regular case reviews and team meetings to support continuous quality improvement.
• Assist in conducting comprehensive assessments of members’ medical, social, and emotional needs as necessary.
• Ensure effective coordination of services among healthcare providers, community resources, and support systems.
• Address barriers to care by supporting staff in problem-solving and resource navigation.
• Serve as a liaison between leadership, staff, and external partners to promote integrated care delivery.
• Support staff training, onboarding, and professional development initiatives.
• Maintain accurate and confidential records in compliance with state, federal, and organizational requirements.
• Advocate for members and populations served to ensure equitable access to care and services.
• Participate in program development, implementation, and evaluation activities.

Qualifications

• Master’s degree in Nursing, Social Work, Psychology, or related field.
• Current professional license or certification as required by state regulations.
• Minimum 4–5 years of experience in care coordination, case management, or community health.
• Previous TCM Experience preferred.
• Prior supervisory & leadership experience preferred.
• Strong knowledge of care management models, community resources, healthcare systems, and patient advocacy.
• Demonstrated ability to lead teams, manage performance, and support staff development.
• Excellent communication, critical thinking, problem-solving, and organizational skills.

Work Environment

• Community-based health setting with a combination of in-office, remote, and field-based responsibilities.
• Regular interaction with staff, members, families, and community partners.
• May require occasional evening or weekend work to support staff and members needs.
