Description:

The Registered Nurse (RN) is responsible for providing high-quality, patient-centered care in accordance with all applicable local, state, and federal regulations.

The RN coordinates, implements, evaluates, and directs patient care activities appropriate to the age and needs of patients served and areas of clinical practice, working collaboratively with providers and the interdisciplinary care team to ensure optimal patient outcomes.

Essential Functions

• Provide comprehensive nursing care utilizing the nursing process, including assessment, planning, implementation, and evaluation.

• Perform patient triage, assessments, and data collection in a systematic manner, addressing physiological, psychological, cognitive, cultural, and age-specific needs.

• Assist providers in the delivery of care, ensuring timely and appropriate treatment within the care delivery system.

• Administer medications (oral and injectable), immunizations, and treatments as ordered by healthcare providers.

• Perform venipuncture and collect laboratory specimens as needed.

• Educate and counsel patients and families regarding diagnoses, treatment plans, medications, and preventive care.

• Coordinate and facilitate internal and external referrals to ensure continuity of care.

• Communicate effectively with pharmacies and insurance companies regarding treatment plans, authorizations, and medication approvals.

• Assist in the development and implementation of nursing policies and procedures in alignment with regulatory requirements and organizational standards.

• Utilize evidence-based practice and current standards of care in clinical decision-making.

• Maintain accurate and timely documentation in the Electronic Health Record (EHR), including transmission and retrieval of electronic health information (EHI).

• Demonstrate competency in Health Information Technology to support care delivery.

• Ensure compliance with OSHA standards, infection control protocols, and safety regulations.

• Respond effectively in emergency situations using sound clinical judgment and established protocols.

• Advocate for patient rights, ensuring adherence to confidentiality standards and HIPAA Privacy & Security Rules.

• Apply principles of the Chronic Care Model in care planning and delivery.

• Promote a collaborative, respectful, and culturally sensitive work environment.

• Participate in quality improvement initiatives and identify opportunities to enhance patient outcomes.

• Maintain professional competence through continuing education and self-assessment.

• Ensure a safe and secure environment for patients, staff, and visitors.

Non-Essential Functions

• Participate in special projects, committees, or organizational initiatives as assigned.

• Provide cross-coverage support to other clinical areas as needed.

• Perform additional duties as assigned by the Practice Administrator and or leadership.

Requirements:

• Current and valid New York State Registered Nurse (RN) license and registration (required).

• Current Basic Life Support (BLS) certification from the American Heart Association (required).

• Current Infection Control certification (required).

• Minimum of one (1) year recent RN experience, preferably in an ambulatory care or FQHC setting.

• Strong clinical assessment, triage, and critical thinking skills.

• Knowledge of ambulatory care practices and population health management.

• Ability to work both independently and collaboratively within a multidisciplinary team.

• Excellent organizational skills with the ability to prioritize and manage multiple tasks.

• Effective communication skills, both written and verbal.

• Demonstrated ability to coach, mentor, and support team members.

• Strong conflict resolution and problem-solving skills.

• Cultural competence and ability to interact effectively with diverse populations.

• Proficiency in Microsoft Office and experience with Electronic Medical Records (EMR/EHR) systems.

• Ability to exercise sound judgment and maintain professionalism in high-pressure situations.

• Frequent direct patient contact across pediatric, adult, and geriatric populations

• Exposure to communicable diseases, bodily fluids, and clinical equipment; PPE required

• Ability to lift or move up to approximately 25 pounds

• Visual and auditory acuity sufficient for patient assessment, documentation, and communication

• Fast-paced environment requiring multitasking, adaptability, and time management

• Ability to communicate effectively in person, by phone, and electronically
