SUMMARY:
A Clinical Intervention Center Pharmacist (CIC Pharmacist) reports to the Lead Clinical Intervention Center Pharmacist (Lead CIC RPh) and Pharmacist-in-Charge/Director of Clinical Intervention Center. The CIC RPh is responsible for supporting the daily operations of the Clinical Intervention Center. This position requires assisting the Lead CIC Pharmacist onboard and train new staff and ensure high-quality clinical intervention services. A CIC Pharmacist communicates regularly with nursing centers and providers, assists in training new staff, provides feedback on team performance, and helps implement best practices to support safe, timely, and effective medication therapy management.
ESSENTIAL FUNCTIONS: To perform this job successfully, an individual must be able to perform each essential function satisfactorily. The CIC RPh should be able to perform all functions of a staff pharmacist as well as those functions listed below. Duties are assigned for multiple pharmacies.
- Assist the Lead CIC Pharmacist in observation of daily operations oversight, including workflow monitoring, issue escalation, and staff support.
- Serve as a resource to other CIC pharmacists and technicians
- Assist the Lead CIC Pharmacist with onboarding and training.
- Monitor and follow up on clinical clarifications utilizing eMAR systems, dispensing history, and direct communication with centers.
- Evaluate STAT medication requests to determine their validity and assess whether fulfillment should be managed internally or routed through the TracRx service.
- Clear old pending clarifications from queue based on new orders or lack of response, by profiling or deleting orders.
- Review and approve Change in Prescription suggestions based on payer preference utilizing ECS messages and test claims.
- Review and approve therapeutic interchange suggestions for non-auto TI states.
- Communicate national backorder issues with facilities and determine need for alternative medications or backup pharmacy use.
- Respond to internal and external communications including email requests and RX Verify control withdrawals (as licensed).
- Review emergency CII prescriptions and follow up with centers or prescribers to ensure receipt of compliant documentation.
- Support the Lead CIC Pharmacist in observations that contribute to identifying performance trends, reporting metrics, and suggesting operational improvements.
- Maintain compliance with pharmacy regulations, policies, and procedures
- Perform all duties of a CIC Pharmacist as needed.

- The regular work schedule for this position is approximately 40 hours per week (Monday-Friday) and may require additional hours/overtime including weekends/holidays as necessary and/or on a rotating basis.
- Must regularly lift and/or move up to 20 pounds.

- While performing the duties of this job, the employee is regularly required to talk or hear. The employee is frequently required to stand, bend, and walk.
MINIMUM REQUIREMENTS:
Education & Experience: Doctorate of Pharmacy; Active State Pharmacist license, State license to handle medication; and 1-3 years of pharmaceutical experience; or an equivalent mix of education and experience, 2- 4 years long term care pharmacy experience preferred. Expertise, depth of knowledge and professional interest in applied pharmacotherapeutics is essential.
Computer Skills: Intermediate proficiency with Pharmacy and medication distribution software (Pyxis Medbank, Framework) and general experience with long term care pharmacy software preferred.
Language Skills: Ability to read and interpret documents such as medication documentation, business correspondence and procedure manuals; ability to write routine reports and business correspondence; ability to speak effectively, both in person and by telephone, to customers or employees of the organization.
Mathematical Skills: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals; ability to compute rate, ratio, and percent and to draw and interpret bar graphs; ability to understand and interpret statistical reports, data charts and graphs.
