SUMMARY

The Community Support Counselor (CSC) provides direct care and habilitative support to individuals within the Agency’s supported living, community programs, family support services, and day/vocational programs. Responsibilities include daily care, skill teaching, supervision, and maintaining accurate documentation.

ESSENTIAL DUTIES & RESPONSIBILITIES
  • Maintain continuous supervision and awareness of each client’s location and status.
  • Assist Program Director/Assistant Director in developing and implementing Individualized Plans of Service (IPOS) and Individual Education Plans (IEP).
  • Teach and assist clients with daily living tasks such as hygiene, personal care, meal prep, budgeting, shopping, clothing care, self-management, and community participation.
  • Track and document progress toward client goals.
  • Protect client rights and promptly report any suspected abuse, neglect, or mistreatment.
  • Complete all DDS and Agency incident and medication error reports.
  • Implement behavior support plans and approved emergency procedures when required.
  • Follow therapy recommendations (OT, PT, Speech), including positioning, sensory activities, transfers, and lifts.
  • Plan and engage clients in recreational and community-based activities.
  • Collaborate professionally with clinical staff, families, schools, and external agencies.
  • Maintain accurate and timely service documentation.
  • Attend required trainings, meetings, and maintain all mandatory certifications.
  • Administer medications and provide routine first aid and health support.
  • Maintain cleanliness and organization of vehicles, program spaces, and equipment.
  • Transport clients to assigned activities and programs.
  • Participate in emergency drills per DDS and Agency requirements.
  • Complete and maintain Medication Certification.
  • Manage petty cash and client cash-on-hand responsibly.
  • Perform other duties as assigned.
TEAMWORK EXPECTATIONS
  • Actively participate in staff meetings, in-services, and required trainings.
  • Work cooperatively with all Agency staff and external partners.
  • Communicate professionally with clients, coworkers, guardians, DDS staff, and community partners.
Requirements: COMPETENCIES
  • Successful completion of Medication Certification Program
  • Completion of PMT, CPR, and First Aid training
  • Valid Connecticut driver’s license with satisfactory driving record
QUALIFICATIONSEducation & Experience
  • High School Diploma or equivalent required
  • Minimum of two years’ experience supporting individuals with developmental and/or behavioral challenges preferred
  • Equivalent combinations of education and experience may be considered
Language Skills
  • Ability to read and interpret documents such as medical notes, safety procedures, and reports
  • Ability to write clear and concise reports and correspondence
Mathematical Skills
  • Ability to perform basic math functions using whole numbers, fractions, and decimals
Reasoning Ability
  • Ability to follow detailed instructions and address problems involving multiple variables
Physical Requirements
  • Regularly required to speak, hear, stand, walk, sit, use hands, and reach
  • Must lift/move up to 75 lbs and may need to exert 100+ lbs of force during physical restraints (training provided)
  • Must have adequate close vision and focus abilities
Work Environment
  • Moderate noise levels
  • Travel by vehicle required with exposure to varying weather conditions
