Life can become complicated quickly, especially for families and seniors navigating limited resources, unexpected hardship, health concerns, or changing circumstances. This role helps ensure residents don’t have to navigate those challenges alone.
By building trusted relationships, connecting people to meaningful resources, and providing compassionate support when needs arise, you help residents experience greater stability, independence, connection, and hope.
What You’ll Do
Build Relationships & Provide Compassionate Support
· Develop trusted relationships with residents, particularly those experiencing hardship, transition, illness, or other challenging circumstances.
· Provide proactive support through phone calls, personal connection, and in-home visits.
· Offer guidance and encouragement while helping residents identify practical next steps.
· Create opportunities for connection through resident-focused activities.
Connect Residents to Resources & Services
· Help residents identify and access community resources, in-home support, counseling, mental health services, and other supportive care.
· Serve as a knowledgeable resource for programs and services available to children, families, adults, and seniors.
· Coordinate referrals and follow up to help ensure residents receive the support they need.
· Stay connected to community organizations, programs, and partners to continually expand available resources.
Partner With Community & Mission 1:27 Teams
· Collaborate with Mission 1:27 and Walton Communities teams to identify resident needs and determine appropriate short-term or ongoing support.
· Meet regularly with Community Managers and Lifestyle Coordinators to maintain strong communication and coordinated care.
· Participate in Legacy and Mission 1:27 team meetings, training, and collaborative initiatives.
· Support outreach efforts that encourage residents to participate in community programs, events, and opportunities.
Coordinate Care & Steward Information Well
· Manage referrals and maintain timely, accurate documentation of resident needs, services, and follow-up.
· Help identify residents who may need additional support or intervention and proactively connect with them.
· Maintain appropriate confidentiality, professional boundaries, and follow-through.
· Represent Mission 1:27 with excellence, compassion, and professionalism while following organizational policies and procedures.

Requirements:What You Bring
Required Qualifications
· A genuine passion for serving families and seniors with compassion, dignity, and respect.
· Experience working with individuals or families in a community, ministry, resident services, social services, or other people-focused setting.
· Strong interpersonal and communication skills with the ability to build trust and navigate sensitive or challenging situations.
· Resourceful, dependable, self-motivated, and highly organized with strong follow-through.
· Ability to work independently while collaborating effectively with community teams, families, and partner organizations.
· Basic computer proficiency, including Microsoft Word, Excel, and Outlook.
· Spiritual maturity, active involvement in a local church, and agreement with Mission 1:27’s Statement of Faith and Qualifications.
Preferred Qualifications
· Bachelor’s degree in social services, human services, psychology, education, counseling, or a related field.
· Experience in case management, social work, resident services, family support, senior services, or community-based care.
· Working knowledge of community resources and supportive services for children, families, adults, and seniors.
· Experience coordinating referrals and connecting individuals with community-based resources and services.
· Experience facilitating support groups, workshops, or other resident or community engagement opportunities.
