Job Summary

The care coordinator supports patient care navigation of clinically complex patients in the clinic setting, managing care for the patients of the clinic to promote effective education, self-management support, and timely healthcare delivery.

Education and/or Experience

• Currently licensed as a Registered Nurse in South Dakota.
• Previous experience in medical, surgical or pediatric nursing necessary.
• Previous clinic experience preferred.
• Leadership experience beneficial.

Licensure/Certification/Registration

• Currently licensed as a Registered Nurse in South Dakota.
