Position Description:

  • Reviews all incoming INR results and doses patients medication based off these results and the parameters set forth by the patient’s provider.
  • Communicates directly with providers regarding critical out of range results.
  • Reviews patient records for changes in condition documented by provider that may affect the efficacy of Coumadin. Adjust dosing accordingly in conjunction with the patients provider.
  • Provides initial and ongoing education to patient and / or family member.
  • Acts as resource for the Coumadin LPN, giving instruction as necessary.
  • Communicates with outside agencies, patients and family members.
  • Other duties as assigned.

Requirements:

  • Education/Certification/Licensure
    • Current Licensure as a Registered Nurse in the State of New York.
    • Current Basic Life Support certification.

  • Experience/Skills/Proficiencies:
    • Bachelor’s Degree of Nursing from an accredited nursing school, preferred.
    • Knowledge of medical terminology, anatomy, physiology and pathophysiology.
    • Familiarity with health care systems, regulations, policies, and functions.
    • Understanding of documentation standards.
    • Knowledge of equipment, supplies, and materials needed for medical treatment.
    • Works well in a team setting and demonstrates strong cooperation skills.
    • Skill in reading and following written and oral medical orders.
    • Skill in initiating appropriate emergency procedures.
    • Skill in handling a number of tasks simultaneously.
    • Experience in a physician office, preferred.

Pay Range: $56,000 - $75,000 DOE
