Clinical LPN - Full Time

Under the direction and supervision of the Clinical Supervisor, the Clinical Licensed Practical Nurse uses and practices basic nursing assessment skills, participates in planning, performs interventions in accordance with the care plan and contributes to the evaluation of individualized interventions related to the care plan. Administers medications, implements primary care orders, and evaluates participant’s reported symptoms in-person and/or by telephone. When appropriate provides health education to participant and caregiver in regards to care plan, nursing treatments or medications. Communicates participant’s condition to primary caregivers, primary care provider, and multidisciplinary team. Maintains the participant’s medical record with thorough documentation. Will insure compliance with competency requirements.

Essential Duties and Responsibilities:

1. Performs basic nursing skills within the Licensed Practical Nurse scope of practice regulated by the North Carolina Board of Nursing. Demonstrates knowledge and competence during performance of nursing skills.

2. Functions as a supplemental member of the interdisciplinary team. Attends and participates in interdisciplinary team meetings when requested, communicates participant changes timely, and collaborates on care planning decisions and coordination for delivery of nursing care.

3. Administers prescribed medications and treatments in accordance with nursing standards, adhering to the five basic rights of proper medication administration.

4. Performs venipuncture procedures, as well as sputum, stool, or urine specimen collection from participants as instructed. May assist with preparation of specimens for transport to lab for analysis.

5. Maintains electronic medical record of each participant and fulfills EveryAge PACE charting and reporting requirements.

6. Observes and records participant’s condition and reaction to medications and treatments and reports observations to primary care provider when necessary.

7. As delegated, assists in documenting verbal orders, ordering medications from pharmacy, and disseminating orders and instructions to appropriate personnel. Implements orders written by primary care provider and nursing orders when necessary.

8. Provides instruction to participants and/or family members regarding prescribed medications and treatments, as assigned by the registered nurse or primary care provider.

9. Provides education and training on care and treatment issues of the frail and elderly as assigned by the registered nurse or primary care provider.

10. Administers and documents participant vaccinations and tuberculosis skin testing.

11. Prepares equipment and assists primary care providers during examination, treatment, and testing of participants.

12. Responds to incoming calls in clinic and conveys telephone messages to appropriate staff timely.

13. Assists with maintenance of the exam rooms for necessary supplies and materials. Ensures cleanliness of clinic environment.

14. Communicates effectively in interdisciplinary team meetings, clinic meetings, or any other meeting as assigned.

15. Assists in the scheduling of internal clinic and external specialist appointments as delegated and assists in care transition follow-up.

16. Monitors medication compliance, administers medications and provides training in self-administration of medications as needed.

17. Provides on-call coverage on a rotating basis with other clinical staff to troubleshoot, advise, and coordinate participant care outside of normal business hours.

18. Conducts visits to participant’s home as needed to assess participant and/or to provide nursing care.

19. Participates in and supports quality improvement initiatives.

20. Maintains a safe working environment, following Carolina SeniorCare’s policies and procedures as well as Occupational Safety and Health Administration (OSHA) safety guidelines.

21. Maintains the confidentiality of all organization procedures, results, and information about participants, clients, or families.

22. Handles potentially infectious specimens with appropriate biohazard precautions, and practices standard precautions during clinical care.

23. Participates in continuing education classes and any required staff and training meetings. Maintains professional affiliations and any required certifications.

24. Assists with preparation of medication planners to promote medication adherence at home.

25. Provides wound care and treatments as ordered by primary care provider.

26. Completes post-fall participant assessments and collaborates with interdisciplinary team regarding fall reduction interventions.

27. Assists with monitoring of participants who are prescribed anticoagulant therapy. Notifies primary care provider timely of pertinent lab results and conveys instructions regarding prescribed anticoagulant dosage titration to participant/caregiver.

28. Performs other duties as required or requested by Clinical Supervisor.

Minimum Education:

Graduate of a school of professional nursing.

Current North Carolina Licensed Practical Nurse (LPN) License,

Cardiopulmonary Resuscitation (CPR) certification required.

Minimal Experience:

Minimum of one year of experience providing skilled nursing care.

One year of working with the elderly
