Procedure:

  • Reporting Responsibilities-The Pharmacist in Charge is responsible to the Owners Managing Officer of the pharmacy and to the Hospital Administrator.
  • Responsibilities-The Pharmacist in Charge will be responsible for executing all functions, services and activities relating to the pharmacy. If absent or unavailable, a pharmacist on duty will assume responsibility for the essential activities and duties of the department.

Responsibilities of the Pharmacist in Charge include:

Professional:

Participating in all company functions requiring the professional knowledge and expertise of a pharmacist.

Communication between the pharmacy and other departments and services.

Ensuring adequate pharmacy staffing for any absences (planned or unplanned) and for the on-call pharmacy services.

Education of all clinical professional staff regarding new drugs, new drug alerts (Black Box Warnings and other reported adverse effects), changes in pricing and all other relevant information.

Standards:

Ensuring that the pharmacy meets the standards of all agencies that license, accredit, or certify the pharmacy and/or the facility.

Ensuring that the pharmacy operates in accordance with accepted ethical and professional standards.

Certify that I hold a Louisiana pharmacist license that is currently in active status and with no restriction that would prohibit my acceptance of an appointment as the PIC of a pharmacy permitted by the Louisiana Board of Pharmacy.

Certify that I have actively practiced pharmacy for at least two years within the jurisdiction of any state board of pharmacy within the United States.

Agree to be present and practicing within the pharmacy for at least twenty hours per week during the pharmacy’s ordinary course of business, or in the alternative, at least 50 percent of the normal business hours where the pharmacy’s normal hours of business are less than twenty hours per week.

Agree to accept responsibility for the complete supervision and management of the prescription department, and for the operation of the prescription department in compliance with all federal and state laws and rules pertaining to the practice of pharmacy.

Agree to accept responsibility for the adequate protection and security of the prescription department.

Agree to accept responsibility for the procurement, receipt, storage, distribution, control, and accountability of drugs for the prescription department.

Agree to accept responsibility for the maintenance of accurate and readily retrievable records in the prescription department for the acquisition, inventory, and disposition of drugs.

Drug Purchasing and Storage:

Ordering, Receiving and Inventory of all medications for the pharmacy

Maintaining an adequate supply of drugs and devices.

Ensures drugs are properly stored at appropriate levels, throughout the institution.

Drug Distribution:

Execution of safe medication practice for ordering, controlling, and dispensing drugs and devices.

Pharmacy Records:

Maintains accurate and complete records of drugs received and dispensed and supplies issued to units, departments, or services as

required by the board of Pharmacy or any other local, state, or federal agency.

Maintains proper records of medication inventory for all state, federal and company required inventory counts.

Maintains all controlled substance inventory is kept perpetually updated for the pharmacy department.- Maintains proper recording and reporting of all pseudoephedrine transactions for the pharmacy department.

Orientation, Education, Training, and Competence

Providing drug information and new product information to the professional staff.

Maintains CE records for all staff pharmacists

Ongoing professional education, including in-service, training and other activities is designed to maintain and improve the competence of the staff.

Orientation provides initial job training and information to any new employees.

Ensure that each registered pharmacists and certified technicians licenses are renewed each year. A copy of the renewed license must be maintained in the pharmacy (and corporate office) and it must be verified as valid and current with the issuing agency. A copy of the verification must be filed.

Requirements:

A valid Louisiana State Board Of Pharmacy License.
