Sangre de Cristo Community Care is seeking a Part-Time Registered Nurse to join our dynamic Home Health team in Cañon City, Colorado. This role offers the opportunity to provide high-quality, compassionate care while working alongside a supportive, mission-driven team dedicated to serving communities across Southern Colorado.

Schedule & Pay

• Part-time position
  
• 3 days per week | 8 hours per day
  
• Starting pay: $35–$38/hour (based on experience)

About the Role
As a Home Health Registered Nurse, you will play a vital role in coordinating and delivering patient-centered care in the home setting. You’ll use your clinical expertise and judgment to support patients, families, and the interdisciplinary team in achieving the best possible outcomes.

Key Responsibilities

• Plan, coordinate, implement, and evaluate nursing care for assigned patients
  
• Assess patient needs and coordinate services and referrals to support individualized care plans
  
• Collaborate closely with the interdisciplinary team to ensure continuous, comprehensive, and responsive care

• A valid non-restricted RN license in the state of Colorado is required.
• Must have a valid non-restricted driver’s license.
• Flexibility and availability to take call for home health and hospice patients.
• At least 1 year of home health or hospice experience preferred.
