Transplant Patient Navigator – RWJBarnabas Health

At RWJBarnabas Health, Transplant Patient Navigators become part of a purpose-driven team committed to compassionate, coordinated, and exceptional patient care. This role provides essential support to transplant nurse coordinators, physicians, patients, and donors throughout the pre-transplant evaluation and transplant process. As a Transplant Patient Navigator, you will help coordinate complex patient care needs, facilitate communication across the transplant team, and ensure that required clinical and administrative information is complete, accurate, and available when needed.

A typical day for a Transplant Patient Navigator may include:

• Coordinate pre-transplant evaluation and ongoing care activities for living donors and transplant candidates/recipients under the direction of the clinical team.
• Coordinate and schedule diagnostic testing, procedures, consultations, and follow-up appointments across RWJBH sites.
• Prepare and maintain patient charts, ensuring required clinical information, test results, forms, and documentation are complete and available for review.
• Obtain, track, and communicate laboratory and diagnostic results to the appropriate RN, physician, or clinical team member and document results as directed.
• Maintain accurate patient registration and referral information, including demographic, financial, authorization, and clinical data.
• Manage transplant-related communications with patients, caregivers, referring physicians, consultants, affiliates, and internal providers and staff.
• Support patient follow-up and care coordination, including follow-up calls, scheduling, insurance authorizations/pre-certifications, and monitoring patient lists for required follow-up or specimens.
• Maintain accurate transplant databases and medical records, ensuring timely and complete documentation of patient activities and results.
• Ensure the integrity and regulatory compliance of medical records, including appropriate patient identifiers, documentation standards, and required care summaries.
• Serve as a knowledgeable, responsive, and compassionate resource for patients, caregivers, providers, and staff while maintaining confidentiality and professionalism.
• Assist the transplant team with designated patient-care activities and program operations, including blood draws/vital signs as appropriate, special projects, and continuous quality improvement.
• May perform other duties as assigned or directed by management

• This position offers a consistent Monday through Friday 8:00am - 4:00pm schedule, allowing you to build strong patient relationships while maintaining a work-life balance.
• An Associate’s degree or Bachelor’s degree is preferred
• Phlebotomy certification is preferred
• One to three years of related experience within the healthcare field is required
• Knowledge of medical terminology, ability and interest to learn clinical information and criteria is required
• Demonstrated computer literacy and strong communication skills, including proficiency with Microsoft Word and Excel is preferred
• Successful completion of all required training and orientation programs
