Care Transition Manager - Social Worker

Work location: Texas Health HEB, 1600 Hospital Parkway, Bedford, TX 76022
Work hours: Full Time - Days; Monday - Thursday; 10-hour shifts, 10:00am - 8:30pm

What You'll Need
In addition to the qualifications, a successful Care Transition Manager - Social Worker will model the factors listed below:
Working knowledge of medical necessity criteria preferred
Knowledge of Microsoft Outlook and Office (Word, Excel)
Customer service skills
Ability to engage in complex clinical decision-making
Strong oral and written communication skills
Strong commitment to interdisciplinary collaboration
Critical thinking, analysis and conflict resolution skills
Psychosocial and crisis intervention skills
Ability to prioritize and meet deadlines

What You'll Need
- Master's Degree in Social Work is required
- 3 years of Social Work experience in a hospital/medical environment is highly preferred
- Case Management / Social Work experience in a hospital setting is highly preferred
- Case Manager / Social Work experience in a post-acute care facility helpful
- 1 year of discharge planning/care management in a hospital setting is preferred
- LMSW - Licensed Master Social Worker is required upon hire or a LCSW - Licensed Clinical Social Worker is required upon hire
- CPR - Cardiopulmonary Resuscitation (BLS) is required upon hire
- ACM - Accredited Case Manager OR CCM - Certified Case Manager is preferred upon hire
