Job Description
The RN Patient Navigator facilitates a timely, integrated and coordinated approach to the care of the patient who has been recently diagnosed with cancer. Acts as a liaison between the patient, staff and physician offices, and ancillary areas of the health system and community. Coordinates physician referrals for patient and/or families to assure patient is seen in a timely manner to discuss potential treatment options. Develops, presents, coordinates and evaluates education activities. Performs in the role of clinician, facilitator, advocate, consultant, and/or researcher. Develops and evaluates a plan of care for the patient during treatment, and survivorship post treatment. Requires flexible schedule and ability to accompany patient to various appointments, such as physician offices, diagnostic testing.
JOB SPECIFICATIONS
Education: Must be a graduate of an accredited school of nursing and have completed a current ONS chemotherapy/ biotherapy course. Must have a BSN or be currently enrolled in a BSN program. RNs who are not currently enrolled must enroll within six (6) months of hire and complete the BSN within three (3) years.
Licensure/Certification: Valid, unrestricted RN license for the State of Ohio. Current BLS required. ONS Chemotherapy/Biotherapy certification required, or must obtain within 6 months of hire. Certification/education in patient navigation from an approved program, or must obtain within 6 months of hire. ACLS preferred. Must obtain within a year. Oncology Certification (OCN) required, or must obtain within 18 months of hire. A valid Ohio driver’s license with reliable transportation required.
Experience: Recent experience in chemotherapy administration and/or radiation required. Previous experience with clinical research, experience in facilitating patient support groups, previous outpatient nursing and/or previous medical-surgical nursing experience are preferred.
Skills: Must have demonstrated knowledge of professional nursing theory and practice to provide and evaluate patient care; excellent interpersonal and “customer service” skills, including the ability to exhibit patience and tact with all constituents including physicians, patients and associates; and the documented ability to work independently with limited direct supervision. Must be able to multi-task and handle stressful situations; have the documented ability to interpret, adapt, and apply guidelines and procedures; and the ability to multitask and organize detailed information. Must have basic computer skills with Microsoft Office products, effective oral and written communication skills, and the ability to shift priorities quickly and calmly to meet patient/family needs.
