The Health Information Management (HIM) Clerk/Document Imaging & Release of Information Specialist is responsible for maintaining the integrity, accuracy, confidentiality, and accessibility of patient health information. This position performs document imaging, release of information (ROI), medical record maintenance, scanning, indexing, customer service, switchboard support, and related HIM functions. The incumbent ensures compliance with applicable federal and state regulations, organizational policies, and HIPAA privacy requirements while providing exceptional customer service to patients, providers, and staff. This position serves as a role model for ethical behavior and promotes an awareness and understanding of positive ethical and moral principles consistent with the Mission, and Behavior Standards of SRMC.

Responsibilities:

· Scan, index, and validate patient documentation into the Electronic Health Record (EHR) accurately and timely.

· Maintain document imaging workflows to ensure records are available for patient care and business operations.

· Review scanned documents for completeness, quality, and proper indexing.

· Ensure all records are scanned according Scans, indexes, and validates patient documentation into the Electronic Health Record (EHR) to departmental policies and retention standards.

· Assist with maintenance of the Master Patient Index (MPI) and medical record number registry.

· Identify and report duplicate medical record numbers and patient demographic discrepancies.

· Maintain and organize completed medical record storage areas.

· Process and respond to patient, physician, third-party, legal, insurance, and governmental requests for health information.

· Maintain release of information logs and tracking systems.

· Process subpoenas, court orders, disability requests, insurance requests, and other legal documents according to policy and HIPAA regulations.

· Validate patient authorizations and ensures proper documentation prior to release.

· Maintain ROI work queues and ensures requests are completed within established turnaround times.

· Document all disclosures in accordance with organizational policies and regulatory requirements.

· Educate patients on accessing medical records through the patient portal.

· Assist patients with portal navigation and obtaining copies of records.

· Answer and direct incoming HIM telephone calls.

· Provide excellent customer service when interacting with patients, providers, attorneys, insurance companies, and staff.

· Assist with walk-in requests related to medical records and release of information.

· Provide daily coverage for the HIM Dept main phone,

· Coordinate switchboard operations with Patient Financial Services (PFS) staff as needed.

· Route calls appropriately and ensures timely response to inquiries.

· Assist with photocopying, faxing, mailing, and other clerical duties.

· Maintain departmental files and records.

· Other duties as assigned.

Requirements:

· High school graduate or GED.

· Knowledge of HIPAA and patient confidentiality preferred but not required.

· Knowledge of medical record documentation practices preferred but not required.

· Ability to manage multiple priorities and meet deadlines.

· Strong organizational and customer service skills.

· Excellent verbal and written communication skills.

· Proficient computer and data entry skills.

· Ability to maintain accuracy and attention to detail.

· Ability to work independently while functioning effectively as part of a team.
