Description
Endeavor Health Services is seeking a compassionate, organized, and community-focused Social Care Network (SCN) Navigator to join our team. In this role, you will support Endeavor's participation in the Western New York and Finger Lakes Region Social Care Networks (SCNs) through the New York State 1115 Waiver. You'll work directly with individuals to identify social needs, connect them with community resources, and help improve access to essential services that promote overall health and well-being.

What You'll Do
As a Social Care Network Navigator, you will:
- Conduct Social Determinants of Health (SDoH) screenings using a standardized assessment tool.
- Provide person-centered navigation services to connect individuals with community resources and support services.
- Meet with individuals in person, by phone, or through secure virtual platforms.
- Identify barriers related to housing, food insecurity, transportation, healthcare access, and other social needs.
- Coordinate referrals and collaborate with healthcare providers, community organizations, and internal staff to ensure seamless, coordinated care.
- Maintain accurate and timely documentation in the Findhelp platform and internal systems.
- Meet or exceed monthly productivity expectations of 80 billable SCN service hours.
- Participate in regional trainings, meetings, and quality improvement initiatives related to the NYS 1115 Waiver.
- Provide coverage for fellow Navigators as needed and participate in agency committees and meetings.
Qualifications
Required:
- Experience in case management, care coordination, community resource navigation, or a related social services role.
- Valid driver's license with an acceptable driving record.
- Reliable access to a personal vehicle with the ability to travel throughout Western and Central New York.
- Proficiency with Microsoft Office applications.
Preferred:
- Bachelor's degree in Social Work, Human Services, Public Health, Psychology, or a related field (all levels of education will be considered).
- Experience using standardized assessment tools.
- Familiarity with Medicaid and the New York State 1115 Waiver.
