This role supports injured workers through coordinated, structured care management within the workers’ compensation system, ensuring continuity of treatment and efficient recovery pathways.
You will work under clinical supervision to coordinate medical services, facilitate communication between all stakeholders, and help ensure timely access to appropriate care.
The position plays an important role in supporting return-to-work outcomes by aligning care plans, provider communication, and employer coordination.
You will manage documentation, scheduling, and case tracking while ensuring accuracy and compliance with established protocols and guidelines.
The environment is fast-paced and highly structured, requiring strong organization, attention to detail, and effective communication across multiple parties.

Accountabilities:
  • Coordinate medical care services for injured workers with low to moderate complexity workers’ compensation claims under established clinical protocols.
  • Facilitate communication between injured workers, healthcare providers, employers, and claims adjusters to ensure continuity of care.
  • Schedule and manage medical appointments, diagnostics, and related healthcare services in alignment with treatment plans.
  • Maintain accurate and timely case documentation, including clinical updates, progress notes, and case management records.
  • Support return-to-work initiatives by tracking treatment progress and coordinating with employers and providers.
  • Monitor adherence to care plans and escalate cases to supervising RNs when clinical complexity exceeds scope.
  • Ensure cost-effective care coordination by supporting the use of in-network providers and established guidelines.
  • Participate in team meetings, training sessions, and ongoing process improvement activities.
Requirements:
  • High school diploma required; completion of an accredited practical or vocational nursing program required.
  • Active and unrestricted LPN/LVN license in a compact state.
  • 2+ years of clinical experience in a healthcare setting.
  • Experience in workers’ compensation, disability management, or care coordination strongly preferred.
  • Strong communication, organizational, and interpersonal skills with the ability to manage multiple cases simultaneously.
  • Proficiency in Microsoft Office (Word, Excel) and experience with data entry and case management systems.
  • Solid understanding of confidentiality standards and HIPAA compliance requirements.
  • Ability to work independently while recognizing when to escalate clinical concerns appropriately.
