Patient Care Coordinator


Position Summary


The Patient Care Coordinator serves as a central point of coordination for patients receiving specialty gastrointestinal care within a gastroenterology practice. This role combines responsibilities traditionally associated with a biologics coordinator, clinic lead, and patient access coordinator to ensure seamless patient care, medication coordination, scheduling, and communication between providers, patients, infusion services, and pharmacy teams.


The Patient Care Coordinator is responsible for facilitating specialty medication workflows, supporting patient treatment adherence, coordinating appointments and procedures, triaging patient concerns, and promoting efficient clinic operations while delivering exceptional patient-centered service.


Essential Duties and Responsibilities


Patient Coordination


• Coordinate care for gastroenterology patients receiving specialty therapies, including biologics and infusion treatments.


• Serve as a primary point of contact for patients regarding treatment plans, medication coordination, and follow-up care.


• Assist patients in navigating prior authorizations, medication approvals, financial assistance programs, and specialty pharmacy processes.


• Monitor patient treatment timelines to ensure timely medication administration and continuity of care.


• Maintain accurate and timely documentation within the electronic medical record (EMR).


Biologics & Specialty Medication Coordination


• Coordinate biologic therapy initiation, maintenance, renewals, and authorizations.


• Collaborate with providers, specialty pharmacies, and infusion centers to facilitate medication access and delivery.


• Track laboratory requirements, infusion schedules, and medication compliance.


• Communicate with insurance companies regarding prior authorizations, appeals, and coverage determinations.


• Educate patients on medication processes, scheduling expectations, and treatment coordination.


Scheduling & Patient Access


• Schedule clinic appointments, infusion appointments, follow-up visits, and diagnostic procedures.


• Coordinate referrals and ensure appropriate follow-up care is completed.


• Maintain provider schedules and assist with optimizing clinic flow and patient access.


• Confirm appointments and communicate preparation instructions to patients as needed.


Triage & Patient Communication


• Respond to patient calls, portal messages, and clinical inquiries in a timely and professional manner.


• Triage patient concerns and communicate urgent or clinical issues to providers appropriately.


• Provide excellent customer service while maintaining confidentiality and professionalism.


• Escalate patient concerns when necessary to ensure safe and effective care delivery.


Clinic Leadership & Team Collaboration


• Act as a clinic lead by supporting daily clinic operations and assisting with workflow coordination.


• Build and maintain strong working relationships with internal infusion teams, pharmacy staff, providers, and clinical personnel.


• Assist with onboarding and training of staff related to biologics workflows and clinic processes.


• Support process improvement initiatives to enhance patient experience and operational efficiency.


• Promote a collaborative, team-oriented environment.

• High school diploma or equivalent required; associate or bachelor’s degree preferred.


• Minimum of 2 years of experience in healthcare coordination, gastroenterology, infusion services, specialty pharmacy, or related field preferred.


• Experience with biologics coordination, prior authorizations, or specialty medications strongly preferred.


• Knowledge of medical terminology, insurance processes, and EMR systems.


• Strong organizational, multitasking, and communication skills.


• Ability to work independently while managing multiple priorities in a fast-paced environment.


• Excellent customer service and interpersonal skills.



Preferred Skills


• Experience in gastroenterology or chronic care management settings.


• Familiarity with infusion scheduling and specialty pharmacy operations.


• Understanding of insurance authorization and appeals processes.


• Leadership experience or demonstrated ability to coordinate team workflows.


Physical Requirements


• Prolonged periods of sitting and computer use.


• Ability to communicate effectively in person and by telephone.


• Occasional standing, walking, and lifting up to 15 pounds.


Work Environment


• Clinical office and healthcare setting.


• Frequent interaction with patients, providers, pharmacies, infusion centers, and insurance representatives.
