Description: Position Overview

Great Lakes Senior Living is seeking a dependable, organized, and compassionate Care Coordinator to support our community’s wellness operations and help ensure residents receive consistent, high-quality care.

The Care Coordinator works closely with the Lead Care Coordinator and assists with resident care coordination, medication processes, documentation, medical supplies, alarm systems, and quality-monitoring activities. This position also works alongside the Resident Assistant/CNA team and may provide direct resident care as needed.

Responsibilities
  • Assist with resident call pendants and bed, chair, motion, and door alarms.
  • Install, activate, inspect, troubleshoot, replace, and remove alarm equipment as needed.
  • Review ECP documentation and identify incomplete care tasks or documentation concerns.
  • Report documentation concerns to the appropriate team member for correction.
  • Assist with monthly medication cycle fills and communicate discrepancies with the pharmacy and leadership.
  • Order PRN medications, narcotics, bulk supplies, and other items as directed.
  • Inventory and organize diabetic supplies and medical products.
  • Assist with the destruction of expired or discontinued medications according to company procedures.
  • Maintain clean and organized medication carts, wellness rooms, and supply areas.
  • Monitor sharps containers and coordinate replacements.
  • Prepare quarterly psychotropic medication review documentation for physician review.
  • Participate in caregiver and Falls Committee meetings.
  • Communicate medical supply needs to leadership.
  • Accurately document completed wellness tasks.
  • Assist Resident Assistants/CNAs and provide resident care when needed.
  • Follow company policies and applicable DHS 83 and DHS 89 requirements.
  • On-call responsibilities
  • Complete other duties assigned by community leadership.

Requirements: Qualifications
  • High school diploma or equivalent required.
  • Experience in senior living, healthcare, medication management, or wellness operations preferred.
  • CBRF training in First Aid and Choking, Fire Safety, and Standard Precautions preferred.
  • Experience working with older adults preferred.
  • Basic computer skills and experience using electronic health records are preferred.

  • Ability to occasionally lift or move up to 50 pounds.
