We're seeking a compassionate and skilled Speech Language Pathologist (SLP) to join our team and make a meaningful difference in the lives of patients recovering from illness, injury, or managing chronic conditions.
As a Home Health SLP, you'll evaluate, develop, and implement individualized treatment plans that improve communication, cognition, and swallowing while helping patients maintain their independence and quality of life.
What You'll DoAs a Speech Language Pathologist, you will:
- Evaluate patients with speech, language, cognitive, voice, and swallowing disorders.
- Develop personalized treatment plans based on comprehensive clinical assessments.
- Treat patients with dysphagia, aphasia, cognitive impairments, voice disorders, fluency disorders, and other communication challenges.
- Perform swallowing assessments and recommend therapeutic exercises, dietary modifications, and compensatory strategies.
- Educate patients and caregivers on communication techniques, home exercise programs, and safe swallowing practices.
- Recommend and train patients on alternative or augmentative communication systems when appropriate.
- Collaborate with physicians, nurses, therapists, and other interdisciplinary team members to ensure coordinated, high-quality patient care.
- Document evaluations, treatment plans, patient progress, and outcomes accurately and in compliance with agency and regulatory standards.
- Participate in case conferences and interdisciplinary meetings to support exceptional patient outcomes.
Required:
- Current Speech Language Pathologist (SLP) license in the applicable state
- Current CPR certification
- Valid driver's license
- Reliable transportation with current auto insurance
Preferred:
- Minimum of one year of rehabilitation or speech therapy experience
- Home Health experience
- Long-Term Care experience
- HomeCare HomeBase (HCHB) experience
- VitalStim Certification
