DCSI Community Health Worker Position Summary

The DCSI Program Community Health Worker (CHW) is responsible for improving patient engagement, addressing social determinants of health (SDOH), and supporting care coordination across Hospital, Urgent Care, and Primary Care settings. The CHW serves as a liaison between patients and the healthcare team by identifying barriers to care, connecting patients with appropriate resources, promoting the use of digital health tools, and supporting positive health outcomes through follow-up and patient education. This position plays a critical role in improving access to care, enhancing the patient experience, and supporting the organization's population health initiatives.

Education:

• High School diploma is required.

Experience:

• Previous experience as a CHW and knowledge of community resources highly desirable.
• Experience in customer support, health care advocacy and outreach is preferred.
• Experience educating and engaging patients preferred.
• Knowledge of local community resources preferred.

Schedule:

• Monday – Friday day shift.
• May require evenings and weekends to meet the needs of the patient.
