Mental Health Provider in Chadron, Nebraska at Chadron Community Hospital, Corp.

Part-time, 20-29 hours/week or PRN/Casual status, 0-19 hours/week.

Hours will be based within Monday-Friday between 8am-6pm.

Benefited position after 60 days waiting period for part-time.

Job Description:

Identify, triage, and manage patients with medical and behavioral health problems within the primary care setting; provide skills training through psychoeducation and patient education strategies. Is responsible for providing comprehensive, integrated mental health care to patients in the community.

PRIMARY RESPONSIBILITIES AND DUTIES:

· Provide counseling/assessments with diagnosis (both in person and via telehealth) for WCHR patients referred by providers and WCHR programs.

· Participate in Clinical Behavioral on-call coverage schedule.

· Conduct psychosocial evaluations and provide therapeutic interventions including crisis counseling to individuals, groups, and families as needed.

· Clinical assessments, treatment, case management, consultation, referral, and education according to the individual’s professional discipline.

· Evaluate the individual and family systems as well as assess client’s functioning within work, family and routines of daily living and identifies areas needing continued support, resources and treatment in order to assist clients.

· Work with primary care teams to treat and manage patients with emotional and health problems, efficiently and effectively.

· Develop, implement, and maintain partnerships with other community providers.

· Teach patients, families, and staff techniques to enhance their care, prevention, and treatment.

· Assess the mental status of patients referred by WCHR staff and primary care providers.

· Provide substance abuse screening, education, and brief interventions.

· Maintain case records to reflect course of treatment and provide required documentation.

· Provide follow-up and aftercare planning services as needed.

LICENSURE/CERTIFICATION:

• Current Nebraska license is required. Licensed Independent Mental Health Practitioner (LIMHP).
• Will consider Provisional Licensed Mental Health Practitioner (PLMHP)
