Job Overview
The Digital Health Navigator (DHN) is a patient-facing guide who helps AltaMed patients and their families successfully understand how to download and use digital tools that facilitate an easier approach to redetermination. The DHN delivers patient education, hands-on digital onboarding, technical assistance, and ongoing follow-up to ensure patients understand the renewal process, maintain their Medi-Cal coverage, and feel confident using AltaMed's digital services. The role also promotes adoption and utilization of AltaMed's digital health platforms, identifies and addresses barriers to digital access, and tracks the learning so we have a better understanding of our patient population with respect to digital tools. The DHN manages an assigned panel of patients, provides personalized outreach throughout the renewal process, and may facilitate group education sessions for patients and health plan members with limited digital literacy. The DHN remains an advocate and supportive liaison with these patients and families beginning with the pilot in fall of CY 26, and during the first two cycles of renewal in CY 27.
