About This Position

  • Job Summary
    • Under the general supervision of the Office Manager, the Nursing Intake Specialist provides services related to the initial intake of the admission process, gathering information which enables the agency to determine the patient has a qualified need for admission to the home health program. Assumes the day to day responsibility for receiving information and coordinating the paraprofessional’s schedule. Also functions as the primary contact for referral services. Exhibits the F.I.R.S.T. values (Friendliness, Innovation, Respect, Service, and Trust).
  • Working Conditions, Hazards and Physical Effort
    • Works in a well-lighted, ventilated and air conditioned area when in the office. Able to properly use telephone, computers, and other office equipment. Schedule may consist of weekends, holidays and shift work. Excellent observation, communication, and problem solving skills as well as basic math skill and nursing skills required.

      Physical endurance for prolonged periods in a stationary position and moving around the unit are required. Must be able to move, position, and/or transfer supplies and equipment. Must be able to move or position 25 lbs. independently.

  • Minimum Qualifications or Equivalents
    • Education: Graduate of an accredited LPN program or an RN program.
    • Experience: Two (2) years of experience as a nurse.
    • EQUIPMENT USED/SPECIAL SKILLS REQUIRED: Word processing computer skills are required. Utilizes communication skills with a high degree of effectiveness in working with patients, families, physicians, hospital personnel and referral agencies. Possesses skills necessary to work effectively with other disciplines. Should possess knowledge of community, social, health and welfare programs.
  • Licenses, Certifications, and/or Registrations
    • Current nursing license in the state of Kentucky. Basic Life Support certification (successful completion within three [3] months of employment).
