Assessment and treatment in a primary care setting with particular attention to chronic diseases. Using EMR to order and document appropriately. Using $4 formularies prescribe medications. Assist with health education. Co-mange care with appropriate specialists.

Core Job Functions
-
Order labs, imaging, diagnostic, medicines appropriately with focus on cost and first step of action, rather than most expensive first. Sends in prescriptions and refills in a timely manner.

Review results of orders in a timely manner and uses results to plan care. Instructs patients on the results, plan of care and outcome expectations.

Investigates thoroughly before requesting specialists. Able to provide referral with all tests, action taken to diagnose and questions regarding irregular outcomes. Willing to co-manage with specialists.

Incorporates SDoH in to the diagnosis and treatment. Documents major barriers that could contribute to the health problems. Refers to Social Worker or others for Community Resources.

Documents appropriately and timely. Researches and consults as needed. Includes the patient in decision making and holds them accountable for agreed outcomes.
