ABOUT CORAZÓN
CPLC Corazón is a client-centered, multicultural center with bilingual staff offering a variety of treatment modalities. We have been providing Substance Abuse treatment through our CARF Accredited Level II Residential Treatment Center since 1983. The 30- to 60-day structured treatment program incorporates best practices targeted to the clients enrolled in the program.
OBJECTIVE AND POSITION SUMMARY
Under general direction, plans, organizes, and provides oversight to LPN nursing staff. Coordinating clinical quality improvement and management, access to care, transfer, triage, nursing assessments, timely documentation, and direct service functions. Ensures that state licensing regulations ADHS and accreditation of CARF standards are being followed. Works as a member of the leadership team, performs direct services as assigned. Serves as community liaison and Corazon representative when assigned. Reports to and is supervised directly by the Chief Medical Officer, with administrative support and collaboration with Program Director.
SPECIALIZATIONS
Have a current license by the Arizona State Board of Nursing as a Registered Nurse (RN)

Complete all clinical documentation according to company standards for services provided including nursing special notes, nursing contact notes and nursing assessments in medication log and electronic health record.
Assist in client admission coordination and documentation to include medications.
Process all incoming medications to include medication log creation and bubble packing of medication.
Coordinate with Corazon staff regarding medical and medication concerns.
Organize system for monitoring client medication adherence.
Report medication non-adherence to Clinical Team.
Complete a nursing assessment and administer and read TB skin test for clients within 7 days of admission.
Assist with urine drug screening in the event that a staff member is unable to support this task.Tests should be performed within testing policies and guidelines.
Coordinate and integrate of care for clients with pharmacies, treatment providers, and other medical professionals.
Actively participate in program activities as directed to enhance client care to include wellness and health education, and medication education.
Provide integrated health education including correlation between mental disorders and medical disorders, effects, and side effects of psychiatric medication, and other medical needs to enhance client health and wellbeing as needed.
Coordinate client care including medication recommendations and assisting clients in coordinating medical and/or other provider appointments as needed.
Help clients in the self-administration of medications and monitoring of this process.
Respond expeditiously in medical emergencies in compliance with company standards and/or best practices, including Incident Reporting procedures.
May perform other duties as assigned.
MINIMUM QUALIFICATIONS AND COMPETENCIES
Education/Background
- Associate’s Degree in Nursing
- Have a current license by the Arizona State Board of Nursing as a Registered Nurse (RN)
- At least 2 years of Nursing experience preferred
- Behavioral Health experience preferred
