The home care office nurse is the lead member of the interdisciplinary team who initially identifies the physical, psychosocial, environmental, and spiritual needs of the patient and family through an admission assessment. A degree in nursing and current license as a Registered Nurse in the state of New Hampshire is required, as well as a minimum of two years of experience in home care, a hospital, or a long term care facility.
