Your Role as a Home Health Physical Therapist

As a Contract Physical Therapist, you will provide skilled therapy services in patients’ homes to help them regain strength, mobility, and independence following illness, injury, or surgery.

Responsibilities include:

Perform in-home physical therapy evaluations and treatmentsDevelop and implement individualized care plansComplete Medicare OASIS documentation, including: Start of Care (SOC), Resumption of Care (ROC), Recertifications, DischargesEvaluationsEducate patients and caregivers on mobility, safety, and recoveryCollaborate with physicians and interdisciplinary care teamsSupervise Physical Therapy Assistants (PTAs) when applicableComplete documentation using AI-assisted EMR tools


Qualifications

Active, unencumbered Florida Physical Therapist licenseOASIS documentation experience requiredCPR certificationValid driver’s license, reliable transportation, and auto insuranceProfessional liability insuranceWorkers’ Comp Exemption or Occupational Accident coverageLymphedema certification preferred
