You will act as a key liaison between a provider network and nephrology practices, ensuring smooth adoption and execution of complex care models. Operating in a fully remote environment, this role requires frequent field engagement with up to 50% travel. - Serve as the primary liaison and subject matter expert for provider network practices, ensuring strong relationships and consistent communication across nephrology partners.
- Support the transition to value-based care by explaining, operationalizing, and reinforcing complex programs such as CMS Kidney Care Contracting and payer/practice agreements.
- Act as a trusted advisor to providers by identifying opportunities for improvement through data insights and delivering actionable recommendations.
- Lead onboarding, training, and ongoing education for providers, ensuring timely and accurate dissemination of new information and updates.
- Manage provider issues and escalations, ensuring prompt resolution and high-quality service across all internal and external stakeholders.
- Maintain accurate documentation of provider interactions in CRM systems and ensure compliance with internal tracking standards.
- Execute strategic initiatives, ensuring deliverables are completed on time and aligned with organizational objectives.
- Bachelor’s degree required; advanced degree preferred.
- 5+ years of experience working with physicians, medical groups, or provider networks; nephrology or value-based care experience strongly preferred.
- Strong understanding or direct experience with value-based care programs, healthcare contracting, or care delivery transformation.
- Excellent communication, presentation, and interpersonal skills with the ability to influence both clinical and operational stakeholders.
- Strong analytical mindset with the ability to interpret data and translate insights into actionable provider recommendations.
- High attention to detail, strong organizational skills, and a proactive, independent working style.
- Willingness and ability to travel up to 50% to assigned provider regions.
