Care Coordination Liaison

PRIMARY PURPOSE:

The Care Coordination Liaison ensures that individual’s access to care through effective coordination of Open Access, Critical Time Intervention (CTI) referrals, agency to agency transfers, psychiatric inpatient hospital discharge planning and Behavioral Health Court referrals.

DESCRIPTION OF DUTIES & TASKS:

Under the direction of the Assistant Director, and within the center’s program policies and procedures, the Care Coordination Liaison performs the following functions.

ESSENTIAL FUNCTIONS:

  1. Pre-Admit, screen, communicate and collaborate on patients attending Open Access, with CSR’s, Support Staff, and Clinicians to support daily operations and workflow
  2. Coordinate and communicate psychiatric inpatient hospital discharges to clinical treatment teams and collaborate with hospitals on discharge planning.
  3. Track pertinent information related to hospital discharge patients including kept appointments and conditional discharges and work with treatment teams to ensure continuity of care for hospital discharge patients.
  4. Agency to agency transfers, this would include collaboration with treatment teams, other Community Mental Health centers and obtaining necessary documentation.
  5. Responsible for a caseload of CTI clients and individuals transitioning into services from a psychiatric inpatient stay.
  6. Attend Behavioral Health Court semi-monthly with participants and Behavioral Health Court Team to give treatment updates, determine advancements, graduations, and sanctions for the participants in the program.
  7. Monitor Behavioral Health Court patients in collaboration with their clinical treatment team.
  8. Provides services and case management to new patients through Open Access and patients enrolled in Behavioral Health Court.
  9. Acts as liaison between hospital social workers, behavioral health court team, CTI program, other CMHCs and the clinical teams to ensure effective communication and collaboration.
  10. Document activities, service plans, and required metrics in the prescribed electronic health record, as identified by the agency in a timely manner.
  11. Performs other duties as assigned.

REQUIRED KNOWLEDGE, SKILLS & ABILITIES:

The individual must possess the knowledge, skills and ability noted below or be able to explain and demonstrate how the individual can perform the essential functions of the job, with or without reasonable accommodation, using some combination of skills and abilities. Must also possess the necessary physical capacities, with or without the aid of mechanical devices or other forms of accommodation, to safely perform the primary functions of the job.

  1. Must be able to establish a rapport with the patients, and treat all patients with dignity and respect.
  2. Ability to organize work-load so that his/her clinical and administrative duties are completed in a timely and thorough manner without the need for direct supervision.
  3. Understands the LRMHC mission and applies this knowledge productively to shape internal policies and external contacts.
  4. Must be capable of working closely with other agency staff to expand the knowledge of mental health services both across programs.
  5. Strong verbal and written communication skills, and have the ability to read and analyze data and reports.
  6. Must be able to tolerate periods of heavy workload, to include long working hours and the balancing of conflicting demands
  7. Must be comfortable technology.
  8. Attendance is a requirement of this position.

QUALIFICATIONS:

Education: Bachelor’s or Master’s Degree in mental health counseling, social work, psychology, counseling or another related field is preferred.

Experience: One to five years’ experience in mental health, crisis intervention or medical setting or related position, offering direct mental health services preferred.

License/Certification: License eligibility in NH as an LCMHC or LICSW, preferred. Valid NH driver’s license.

Physical Requirements:

Able to tolerate periods of heavy workload and excessive stress.

Able to sustain light physical effort such as frequent standing, walking, bending, stooping, or reaching and/or occasional lifting of items 20 1bs. or more.
