About This Position
Why Choose Community Inclusion?
At Community Inclusion, we believe that a diagnosis should never define the boundaries of a person’s life. For individuals supported through the Supports for Community Living (SCL) and Michelle P. Waiver (MPW), our mission is to move beyond basic care toward true community integration. We don't just provide services; we build bridges to independence, ensuring that every individual has the opportunity to live, work, and thrive in the neighborhoods they call home.
Our approach to waiver services is centered on the "Diagnosis to Death" philosophy—a commitment to a lifetime of consistent, high-quality advocacy. By focusing on person-centered planning and stable transitions, we empower individuals to achieve their personal goals while providing their families with the peace of mind that comes from a dedicated, long-term partnership. At Community Inclusion, we are not just a provider; we are a lifelong ally in the pursuit of a meaningful life.
