The School-Based Health Center (SBHC) Nurse Practitioner provides school health nurse services and primary care services to students in the assigned schools. The Nurse Practitioner provides school health services, preventive and acute care, chronic care management, and reproductive health care. The Nurse Practitioner coordinates care delivered and is responsible for billing and reporting requirements, including all appropriate documentation with oversight from the Director of Primary Care and a collaborating physician.

The Nurse Practitioner, as the leader of the SBHC, upholds the health center mission by assuring that patients/students receive health care that is competent, caring, cost-effective, and meets with a high degree of satisfaction; and supports the fiscal stability of the organization.

ESSENTIAL DUTIES & RESPONSIBILITIES:

  • Provides a wide spectrum of primary care services to patients/students. Care includes, but is not limited to, medical assessment, diagnosis, treatment, referral and follow-up. 
  • Proficient in the management of acute and chronic conditions, both simple and complex. 
  • Practice is based on accepted evidence-based criteria and established clinical practice guidelines, utilizing an individualized, dynamic plan of care that can be applied across the continuum. 
  • Updates patient/student health records for accuracy at each patient/student encounter using appropriate designated forms and documentation requirements.  
  • Orders, interprets, and evaluates diagnostic tests to identify and assess patients’/students’ clinical problems and health care needs. 
  • Documents/dictates key components of patient's/student’s progress via daily progress note, transfer, and discharge summary, and or clinic note where applicable. Documentation is timely, meets acute care compliance standards and captures patient acuity. 
  • Consultation with collaborating physician and other members of the healthcare team when indicated. 
  • Collaborates with interdisciplinary health care team in making diagnoses of acute and complex chronic conditions and referral of services when applicable, including the student’s primary care provider (PCP) and/or specialist.  
  • Identifies expected outcomes from diagnosis and formulates and documents a plan of care to address health care needs. 
  • Implements and modifies plan of care where necessary. 
  • Prescribes diagnostic strategies and therapeutic interventions when identified. 
  • Maintains ongoing communication and collaboration with interdisciplinary care team. 
  • Facilitates and communicates with patient, family, staff, and PCP to promote continuity of care across the continuum of care. 
  • May perform advanced procedures consistent with privileges and competency validation. 
  • Responsible for the delivery of high-quality care and practice outcomes in nurse-managed school-based health center/s serving low-income student populations. 
  • Provide preventive physical and behavioral health screenings and care coordination as appropriate. 
  • Work in collaboration with a Behavioral Health Consultant and Patient Service Representative. 
  • Maintains and furthers close communication and coordination for SBHC with the principal, school nurse, and/or their designees for optimal school integration.  

EXPERIENCE AND REQUIREMENTS:
Candidates should possess a diverse range of skills and experience including but not limited to:

Required Education and Experience

  • Current RN and APN licenses from the Commonwealth of Pennsylvania. 
  • Master’s degree. Graduate from an accredited nurse practitioner program. 
  • 2-3 years pediatric or family medicine, adolescent health, or emergency experience
  • School-based health experience preferred.  
  • Board Certification as a Nurse Practitioner in pediatric or family medicine, adolescent health experience preferred. 

Required Skills and Abilities

  • Experience serving children and youth from 5-18 years, including reproductive health services. 
  • Current nationally recognized advanced practice certification or in progress. 
  • Knowledge or interest in pursuit of knowledge related to school nurse standards, practice, and law. 
  • HIV and Family planning experience a plus. 
  • Physical and emotional ability to perform the duties and responsibilities associated with the position. 
  • Work and function in a culturally competent capacity with persons from a variety of ethnic and social backgrounds. 
  • Possession of exemplary interpersonal and communication skills with both internal and external customers. 
  • Practice and behaviors reflect fiscal responsibility in appropriate balance with quality patient care. 
  • Defined productivity standards are proactively worked towards and established goals met. 
  • Demonstration of good time management skills and follow through. 
  • Attendance and participation in meetings, mandatory in-services, educational programs, and other activities in order to stay informed and current. 
  • Knowledgeable and supportive of policies, procedures, and guidelines associated with practice setting. 
  • Understanding of psychosocial impacts of health outcomes in underserved communities 
  • Exhibits cultural competency and sensitivity 

Lifting Requirements

LIGHT – Exerting up to 20 pounds of force frequently, and/or negligible amount of force constantly to move objects. The use of arm and/or leg control requires force greater than sedentary, but worker still sits majority of time.
