Who We Are
At Essential Services LLC, we are more than a provider agency. We empower individuals with intellectual and developmental disabilities by connecting them with qualified Direct Support Professionals (DSPs) who provide personalized, in-home and community-based support.
Our Care Coordinators are relationship managers at the center of that process. They keep clients engaged, services moving, communication clear, and relationships strong. Success in this role means being proactive, responsive, solutions-oriented, and comfortable driving conversations toward clear next steps.
About the Role
This is a full-time, in-office position, Monday through Friday from 9:00 AM to 5:00 PM.
As a Care Coordinator, you will manage a portfolio of client and DSP relationships and take ownership of their overall service experience. You will be a primary point of contact, proactively following up, identifying issues, maintaining engagement, and helping ensure successful, long-term service relationships.
This is a fast-paced, customer-facing role for someone who enjoys talking with people, building rapport, influencing positive outcomes, and keeping multiple relationships moving forward at once.
The strongest candidates are proactive communicators who are comfortable picking up the phone, asking the right questions, navigating objections or difficult conversations, and creating clear next steps.
While our work serves individuals with intellectual and developmental disabilities, this is not a direct-care or case-management position. Your role is to manage relationships, coordinate solutions, and drive follow-through rather than personally providing care or taking ownership of responsibilities that belong to the client’s broader support team.
Key Responsibilities
1. Relationship Management & Communication
- Serve as a primary point of contact for clients and DSPs
- Build strong, professional relationships and maintain regular communication
- Guide conversations toward clear solutions and next steps
2. Client & DSP Engagement
- Conduct proactive check-ins to assess satisfaction and service effectiveness
- Follow up on placements, services, and outstanding needs
- Maintain engagement and keep relationships moving forward
3. Service Oversight
- Monitor services and identify concerns or gaps requiring attention
-Respond proactively to issues that may impact client satisfaction or service continuity
- Coordinate with internal teams to implement solutions
4. Conflict Resolution & Accountability
- Address concerns, conflicts, or performance issues in a timely and professional manner
- Document interactions, concerns, and resolutions accurately
- Support accountability by identifying patterns, addressing issues, and escalating when necessary
5. Documentation & Follow-Through
- Maintain accurate, up-to-date records of client interactions and service updates
- Track outstanding items and ensure timely follow-through
- Manage multiple client relationships and priorities effectively
6. Collaboration & Continuous Improvement
- Work closely with internal teams to support successful client relationships
- Communicate updates, changes, and client needs effectively
- Identify opportunities to improve processes, communication, and service quality

Requirements:Preferred Qualifications
- Experience in customer service, sales, account management, recruiting, or another relationship-focused role
- Strong interpersonal skills with the ability to build rapport and maintain professional relationships
- Confident communicator who is comfortable with proactive outreach, follow-up, and difficult conversations
- Ability to identify needs, problem-solve, and guide conversations toward solutions
- Strong sense of urgency, accountability, and follow-through
- Ability to manage multiple clients, priorities, and ongoing relationships in a fast-paced environment
- Strong attention to detail and accurate documentation
- Proficiency in Microsoft Outlook, including managing communication and scheduling
