At Essential Services LLC, we are more than a provider agency. We empower individuals with intellectual and developmental disabilities by connecting them with qualified Direct Support Professionals (DSPs) who provide personalized, in-home and community-based support.
Our Care Coordinators ensure that services remain consistent, effective, and aligned with each client’s needs supporting strong relationships and high-quality outcomes.
About the Role
This is a full-time, in-office position, Monday through Friday from 9:00 AM to 5:00 PM.
As a Care Coordinator, you are responsible for overseeing the quality and consistency of services provided to clients. You will serve as a primary point of contact for clients, Direct Support Professionals (DSPs), and case managers, ensuring communication is clear, services are effective, and concerns are addressed promptly.
This role requires strong communication, sound judgment, and the ability to manage relationships while navigating challenges in real time. You will play a key role in maintaining service quality, supporting client satisfaction, and ensuring accountability across all parties involved.
Key Responsibilities
1. Relationship Management & Communication
- Serve as a primary point of contact for clients and DSPs
- Build and maintain professional, trust-based relationships
- Facilitate clear communication between clients, DSPs, and internal teams
- Provide guidance and support to maintain strong, effective working relationships
2. Client & DSP Support
- Conduct regular check-ins to assess satisfaction and service effectiveness
- Follow up on services, placements, and ongoing needs
- Ensure clients and DSPs feel supported and heard throughout service delivery
3. Service Oversight
- Monitor services to ensure alignment with client needs and support plans
- Identify gaps in care or areas needing adjustment
- Collaborate with internal teams to implement changes and improve outcomes
4. Conflict Resolution & Accountability
- Address concerns, conflicts, or performance issues in a timely and professional manner
- Document interactions, concerns, and resolutions accurately
- Support accountability by identifying patterns, addressing issues, and escalating when necessary
5. Documentation & Reporting
- Maintain accurate, up-to-date records of client interactions and service updates
- Ensure documentation is completed thoroughly and on time
- Support transparency and consistency across all service-related records
6. Collaboration & Continuous Improvement
- Work closely with internal teams to ensure coordinated service delivery
- Communicate updates, changes, and client needs effectively
- Identify opportunities to improve processes, communication, and service quality

Requirements:Preferred Qualifications
- Experience in healthcare, social services, or case management environments
- Experience working directly with clients, families, or support staff
- Strong interpersonal skills with the ability to manage relationships and navigate conflict
- Demonstrated ability to handle sensitive situations with professionalism and sound judgment
- Strong attention to detail in documentation and follow-through
- Proficiency in Microsoft Outlook, including managing communication and scheduling
- Ability to manage multiple clients and priorities in a fast-paced environment
