Pay Range $24.16-$38.65*
Job Summary:
As a Licensed Practical Nurse (LPN), you will assess patients' needs to determine the urgency of their condition and direct them to the appropriate level of care. Whether by phone or in person, you'll collect information about patient symptoms and history, prioritizing patients based on the acuity of their condition. Your role also involves providing clear instructions, coordinating continued care, and accurately documenting all patient interactions. You will project a positive image of our organization with a strong focus on customer service and quality.
Essential Functions

Determine the urgency of a patient's condition based on symptoms, medical history, and injuries to guide them to the appropriate level of care.

Provide clear instructions to patients and families regarding medications, post-operative care, follow-up appointments, and stat exam appointments.

Coordinate STAT patient referrals to specialists, arrange appointments, and manage communication between providers.

Accurately document all patient interactions and treatment plans.

Input physical therapy scripts and coordinate in-home therapy for patients who cannot leave their homes.

Perform accurate PMP (Prescription Monitoring Program) checks for medications being prescribed and refilled.

Maintain professional and interpersonal relationships with providers to effectively communicate patient needs.

Enter diagnoses, order medication refills, place image orders, and write disability slips.

Be flexible and assist in other clinical areas as directed by leadership.
Qualifications

Education/Certification/Licensing:

High school diploma or GED
-
Licensed Practical Nurse License

Current BLS/CPR certificate

Experience:
-
One year of experience in an office or medical setting. Experience in a triage or emergency care setting is preferred.

Excellent communication and interpersonal skills are essential for interacting with patients and providers.
