About This Position

PSESD is seeking a highly skilled Case Manager to support youth and young adults ages 14 to 24 in accessing education and employment services, with a requirement of attaining a secondary school diploma or equivalent, and transitioning into post-secondary education, apprenticeships, or employment. This includes outreach, eligibility screening, assessment, advocacy, and connecting participants to resources, education, and career pathways. The Case Manager also provides job readiness training, places participants in paid internships, and partners with businesses to expand employment opportunities.

This job reports to Director, Math, Science and PreK-12 Systems

Essential Functions

Assists participants in obtaining employment and developing job readiness skills (e.g. skill development, resume assistance, improved interview skills, etc.) for the purpose of helping participants successfully enter and thrive in the workforce.


Assists participants with skills needed (e.g. workplace maturity, time management, communication, conflict resolution, interpersonal relations, job search, etc.) for the purpose of obtaining and maintaining employment, entering post secondary, and/or accessing wage progression opportunities.


Collaborates with other project partners (e.g. colleagues, worksites, businesses, families, and local schools/educational agencies, etc.) for the purpose of coordinating support, expanding opportunities, and ensuring participant success.


Completes and maintains current, accurately documented participant files according to deadlines for the purpose of accurate recordkeeping and file maintenance.


Conducts participant initial screening and comprehensive assessment of job readiness, strengths, and barriers within a holistic, family development model for the purpose of establishing individual education and employment plans with participants.


Connects with community partners to recruit and screen potential participants for the purpose of determining eligibility criteria.


Embodies the principles and practices that form our culture and commitment to becoming an Antiracist Multicultural Organization for the purpose of meeting the Agency’s End: Success for Each Child and Eliminating the Opportunity Gap by Leading with Racial Equity.


Engages in outreach to local community businesses for the purpose of developing and maintaining employment and internship opportunities for participants.


Maintains accurate, up to date employment information and resources for the purpose of promoting job- ready participants to regional employers.


Maintains current working knowledge of risk factors, cultural competencies, post secondary programs, college funding, business techniques, workforce needs and industry trends for the purpose of providing informed guidance and relevant support that aligns with participant goals and labor market demands.


Monitors interns placed with worksite partners for the purpose of ensuring success.


Navigates educational resources and services for the purpose of ensuring participants receive GED/High School Diploma and/or transition to postsecondary education.

Orients new participants into the program for the purpose of establishing program expectations, eligibility, and documentation.


Participates in meetings, workshops, and seminars for the purpose of conveying/gathering information required to perform job functions and/or maintaining relationships with other agencies.


Provides on-going case management for the purpose of ensuring participant success in entering other services and programs.


Provides program participants with support services for the purpose of reducing barriers to employment and ensuring accurate documentation of support services.


Works with participants to reduce employment and education barriers for the purpose of referring and advocating to community resources.


Other Functions

Assists other personnel as may be required for the purpose of ensuring an efficient and effective work environment.


COMMUNITY AND FAMILY ENGAGEMENT: Demonstrates knowledge and expertise related to engaging with stakeholders, including children, families and communities, to promote wellness and self-sufficiency.

DEVELOPING AND EMPOWERING OTHERS: Encourages others to work on challenging assignments that grow their skills and abilities. Supports others in identifying their own strengths and skills-gaps, and guides them in creating their own professional development plans that support current and future Agency goals.

PLANNING, ORGANIZATION, AND PRIORITIZATION: Assesses the work to be performed, and considers how it should be organized and accomplished, with appropriate priorities and realistic time parameters.

PROFESSIONAL DEVELOPMENT FACILITATION: Sharing knowledge and expertise with the goal of improving professional skills and knowledge through adult learning programs.

CHILD GROWTH AND DEVELOPMENT: Demonstrates knowledge and expertise related to child/youth development and growth.

  • Community College and/or vocational school degree in a job-related field; a GED/High School Diploma may substitute with increasing levels of relevant work experience.
  • Two (2) years of experience in case management, workforce development, or related services supporting youth and young adults (ages 14-24).
  • Ability to work effectively with diverse populations, including those facing barriers such as homelessness, domestic violence, and substance abuse.
  • Demonstrates strong organizational, time management, and technology skills with the ability to manage multiple priorities and maintain accurate documentation.
  • Professional experiencing building relationships with businesses, schools, and/or community partners to support education and employment pathways.
  • Ability to work independently and collaboratively in a dynamic environment.

  • Experience diffusing conflict and navigating sensitive situations with a strengths-based approach.
  • Established connections with local community resources, employers, and educational institutions.
  • Experience in workforce development, including labor market trends, job readiness, and career counseling.
  • Knowledge of post-secondary pathways, including GED/High School diploma completion and college/apprenticeship enrollment.
  • Experience facilitating training, workshops, or professional development sessions.
  • Bilingual proficiency is a plus.
