About This Position
Job Summary:

The Patient Care Coordinator is accountable and held responsible for managing the clinical care and course of treatment for several patients. Coordination of care is accomplished through collaboration with physicians, associate staff, and other healthcare providers to ensure that patient outcomes are achieved within the projected course of treatment using the nursing process. The Patient Care Coordinator is accountable to the Director of Emergency Services. They will utilize an appropriate knowledge base and skill set to communicate and interact with patients, families, and visitors of all ages while being sensitive to their cultural and religious beliefs. The coordinator will serve as the director's extension, overseeing the nursing practice in this unit. This job description is not intended to be all-inclusive; the employee will also perform other reasonably related job duties as assigned. |
Performance Expectations:
- Demonstrates the ability to perform the expectations of the ED RN, in all areas.
- Assists the director with evaluations of staff
- Demonstrates ability to supervise staff performance, providing feedback as appropriate.
- Responsible for patient flow in the ED
- Works closely with the house supervisor to ensure an efficient flow of admissions from the ED to the inpatient units.
- Demonstrates the ability to generate a daily staffing schedule according to the approved matrix, and communicate with the manager when unable to do so.
- Utilizes critical thinking abilities and can care for trauma, stroke, SEPSIS, and STEMI patients according to protocol, policy, and procedures.
- Utilizes the ability to collect evidence, following all state, federal, and/or hospital regulations.
- Keeps a clean, safe, and efficient work environment.
- Demonstrates the ability to serve as a role model.
- Utilizes the ability to communicate effectively with staff, managers, physicians, and the executive team
- Demonstrates the ability to promote and support teamwork.
- Possesses ability to review the EMR for abnormal results and communicate with the ERP in further treatment is needed.
- Demonstrate leadership attributes and characteristics to promote efficiency in the department.
- Ability to remain certified and licensed in all required certifications/licensures.
Qualifications:
Education/Skills
Graduate of a recognized non-online RN program.
Work Experience:
Minimum of two years as a staff RN in an acute setting.
Leadership experience preferred.
