We are a newly established hospice organization committed to delivering compassionate, dignified, and personalized end-of-life care to patients and their families in the Milwaukee area. We are seeking a compassionate and experienced Hospice RN Case Manager to join our interdisciplinary care team. As the primary point of contact for your patients and their families, you will manage a caseload, develop individualized care plans, and provide hands-on nursing care focused on comfort and quality of life. Requirements:- Current RN license in Wisconsin (required) from a program accredited by the National League for Nursing.
- Minimum 1 year of hospice or home health experience preferred
- Ability to work independently and collaboratively in a startup environment
- Demonstrated competency in performance of professional nursing functions.
- Requires excellent oral and written communication skills.
- Ability to work with dying patients and their families. Possesses a sympathetic attitude toward caring for the hospice patient and demonstrates positive communication skills in interacting with other members of the hospice team.
- Ability to maintain a high level of confidentiality concerning patient and family information.
- Competent using computers with knowledge of electronic medical records documentation.
- This position requires a personal vehicle to perform the essential job functions. A valid driver’s license including an acceptable driving record, and auto insurance coverage are required. You will be required to travel within the organization’s area of service
Responsibilities & Duties:
The primary responsibility of a Hospice Nurse is to keep patients comfortable during the last few months of their lives. To achieve this overall goal, they may perform the following job duties:
- Perform comprehensive nursing evaluations.
- Assess for signs and symptoms of discomfort including presence of pain and takes prompt and appropriate action.
- Initiate plan of care and appropriate supervises, provide appropriate nursing follow- up to all patients.
- Instructs caregivers in managing patient needs and submits appropriate documentation.
- Coordinate all services provided to patients/families in all settings to ensure quality of care and services as well as appropriateness of care.
- Supervise the implementation of the approved plan of care, modifies as needed, and contacts physician when appropriate.
- Provide supervision of Hospice Aides and LVN/LPN.
- Demonstrate flexibility in scheduling and readily accepts new admissions and emergency assignments as requested.
- Maintain knowledge of state and federal regulations.
- Observe for safety hazards in the patient’s and organization’s environments and takes appropriate steps to reduce or eliminate them.
- Maintain continuing education through academic studies, seminars, and workshops.
- Attend and participate in in-services, staff meetings, and patient care conferences.
- Completes all documentation in an accurate, legible manner and in the timeframe required by standards and regulations.
Working Conditions:
- Environment: Work in patient homes, healthcare facilities, and office settings.
- Travel: Regular travel within the service area; valid driver’s license required.
- Hours: Flexibility to work evenings or weekends as needed.
