About This Position

Under supervision, the Community Health Worker I (CHW I) provides health education, disease management education, health care and social service navigation services, and/or case management services to patients/community residents.

MINIMUM EDUCATION:

  • High school diploma or GED.

MINIMUM WORK EXPERIENCE:

  • One year of health education, health care, or related experience preferred.

KNOWLEDGE & SKILLS:

  • Ability to provide health education, one-on-one and in groups.  Ability to provide case management under supervision.
  • Familiarity with and/or experience with working in Chicago’s West Side and/or South Side communities (resident of West Side or South Side communities preferred).
  • Passion for and dedication to working with community members and providers.
  • Ability to connect and build trust with diverse patient/participant population.
  • Demonstrated commitment to racial and health equity.
  • Punctual, reliable, and willing to learn.
  • Working understanding of Microsoft Word, Access, Excel, and PowerPoint preferred.
  • Valid driver’s license and own car preferred (required for some positions).
  • Bilingual in Spanish may be required for some positions.

**** Hours are flexible but will include some evening and weekend hours scheduled in advance.
