Citizens Memorial Hospital Foundation (CMH) is recruiting a Registered Nurse (RN) to be our MDS Coordinator (RN) -Resident Care Coordinator to care for our residents in our Lake Stockton Healthcare Facility! CMH is made up of 34 primary and specialty clinics, 6 Long-Term Care facilities and 1 residential care facility. Offering acute care, emergency services, ambulatory care, pharmacies, walk-in clinics, surgeries, addiction recovery, therapies and more, CMH has its community covered!
About the area: Stockton Missouri is home to Stockton State Park, which offers a range of outdoor activities, including hiking, biking, fishing, boating, and swimming. Stockton Lake is located a few minutes from our facility and is a popular destination for boating, water sports, and camping!

About the Facility: Lake Stockton Healthcare Facility is a 90 bed, skilled nursing facility in the beautiful lake town of Stockton, Missouri. We appreciate and respect our employees, or as we call them, “Lifeguards”. Our goal is to be the first choice for both care and career by providing the safest, highest quality, and best experience for our residents and Lifeguard team. In addition to providing exceptional care and services; we strive for a fun, positive, and rewarding work atmosphere that is not like the rest!
A typical day includes: working closely with the facility Director of Nursing to ensure staff or residents' needs are met.
- Aiding in Nursing staff development and education.
- Coordinating facility orientation for new nurses.
- Ensuring compliance with State and Federal Regulations.
The position is Full Time, Days
Job Requirements:
- Education: Associate's degree
- Experience: LTC and MDS experience preferred
- Skills: Strong attention to detail
- License: LPN, LVN, RN
- Physical: Ability to sit or stand for extended periods, and move throughout the facility to monitor resident care.
