Position Summary:
Provides direct patient care in Cardiac and Pulmonary Rehabilitation. Monitors patients during exercise sessions. Responsibilities focus on the preventive and restorative healthcare needs of the cardiovascular patient. Provides patient education on disease entities, cardiac risk factors, and behavior modification. Communicates with the Cardiac Rehabilitation Medical Director and the patient's physician about changes in the patient's physician about changes in the patient's clinical condition, including laboratory tests and symptomatology. Is able to respond quickly and accurately to changes in the patient's condition and response to treatment. Participates in performance improvement and QI activities.
Essential Functions:
- Performs head-to-toe assessments on all outpatients and reassessments in collaboration with rehab staff and Medical Directors.
- Develops and revises treatment plan as indicated by the patient's response to treatment and evaluate overall plan at each visit. Plan includes exercise and education goals per American Association Cardiovascular and Pulmonary Rehab (AACVPR) Individualized Treatment Plan (ITP) guidelines.
- Knowledge of medications, treatments and their correct administration based on the age of the patient, adults through the geriatric population, and their clinical condition (within scope of practice)
- Formulates a teaching plan based upon identified learning needs and evaluates effectiveness of learning; family is included in teaching as appropriate.
- Provides care appropriate to condition and age of the patient including the adult and the geriatric population.
- Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/healthcare errors.
- Identifies and addresses psychosocial, behavioral, physical, cultural, ethical and religious/spiritual needs of patients and their families.
- Manages and operates equipment safely and correctly. Provides maintenance and calibration of equipment as applicable.
- Other duties as assigned.
Qualifications:
- Graduate of an accredited school with Associate degree required. Bachelor degree preferred. May include RN, PT, RT, RD, or related field.
- ACLS require within 90 days of hire.
- BLS certification required.
- High integrity, able to follow procedures.
- Ability to be a self-motivated learner, to take initiative in learning the many complex processes.
- Exhibits professionalism at all times, understands appropriate workplace behavior/communications.
- Strong written and verbal communication skills.
- Confidentiality a must.
