Position Overview We are seeking experienced and compassionate Supervisory Registered Nurses (RN) to support our skilled home health team. This role is ideal for nurses who enjoy independent field work, clinical leadership, and delivering high-quality patient care in the home setting.

Responsibilities Perform comprehensive patient assessments Develop and update individualized care plans Provide skilled nursing services (wound care, infusions, medication management, catheter care, patient education) Supervise LPNs, CNAs, and direct care staff Monitor patient progress and communicate with physicians and leadership Complete Medicaid evaluations and re-certifications Ensure compliance with regulatory and agency standards Participate in discharge planning to support safe transitions of care
Qualifications Graduate of an accredited BSN, ADN, or Nursing Diploma program Active DC RN license Minimum 1 year RN experience (acute care/Med-Surg preferred) Home health experience strongly preferred Strong leadership and communication skills Comfortable using electronic health records (EHR)
