Location: 130 Rumsey Street, Bath NY - Weider House

Department: Residential Services

Employment Type: hours will vary with nights, weekends, and on call

Salary: $26.00 an hr

General Description

Under the direction of the Clinical Supervisor, the Senior Counselor will assist with the planning, development and implementation of the daily operations of the community residence. The Senior Counselor functions as a primary counselor in the care of the residents and is responsible to act as the primary supervisor on an as needed basis.

Essential Duties and Responsibilities

  • Ensures all residents receive comprehensive case management services and complete access to community services for legal, mental health, social services, vocational assessments, and misc. needs.
  • Completes phone screens and sends to Program Director for review and approval.
  • Completes admissions for all new residents to ensure all required documentation is completed and documented in client’s chart.
  • Ensures all residents have a medical history recorded and a physical exam performed within 45 days of admission.
  • Ensures the following: comprehensive service plans are prepared within 30 days of the development of the initial service plan, service plans are reviewed at least every 30 days thereafter and signed as required by the supervisor.
  • Ensures the discharge plans provide for the specific needs of a resident and prepared in collaboration with the resident.
  • Ensures progress notes are prepared at least once per week and reflect the goals and objectives of the service plan.
  • Ensures all records are locked and medications are stored and monitored according to program policy.
  • Ensures the adherence of confidentiality regulations.
  • Evaluates residents’ living skills and develops plans to monitor and improve skills.
  • Coordinates services and communicates residents’ conditions to appropriate service providers.
  • Provides counseling, education and training to residents in living skills through individual and group work.
  • Communicates effectively and works cooperatively with residents, staff and community professionals.
  • Participates in the development and maintenance of the therapeutic community.
  • Gets involved in resident’s daily activities, observes behavior and insures the maintenance of appropriate conduct. Provides for the safety and integrity of the facility at all times.
  • Provides crisis management as needed. Has the ability to identify symptoms of decomposition and develop strategies to address concerns with appropriate treatment providers.
  • Maintains professional practices.
  • Adheres to Kinship’s policies and procedures, performs other related tasks as assigned.
  • Completes LOCTDR at time of admission and discharge.
  • Assists with on call as needed to help support the program.
  • Completes HERDS reports daily for supportive living program.

The additional responsibilities of the senior counselor:

  • Supports the clinical supervisor by providing assistance in planning, developing, implementing, and evaluating residence programming and operations.
  • The Senior Counselor provides coverage for the clinical supervisor as needed and while the supervisor is on vacation.
  • The senior counselor achieves or exceeds program standards on performance evaluations.
  • The senior counselor serves as a role model and/or mentor for the other staff and will model patient-centered care and evidenced based practice approaches in service delivery.
  • The senior counselor assists in orientation and training of staff as directed by the clinical supervisor.
  • Completes other duties as assigned.

Requirements:

Qualifications

Education: Associates degree required.

Credentials: CASAC preferred. CASAC preferred. CASAC in training with evident progress in obtaining a CASAC credential required.

Valid NY State Driver License and access to a reliable vehicle

Experience: Minimum two years of experience and education in chemical dependency or human services field. Relevant years of experience and education will be considered

Additional Qualifications:

  • Adheres to all applicable federal and state laws and regulations including, but not limited to, those governing confidentiality, privacy, program, billing and documentation standards. All duties must be performed in accordance with CCDR’s corporate compliance and ethics program.
  • Ability to prioritize assignments, plan, and complete work projects with minimal direction
  • Exceptional time management skills and ability to effectively and meet deadlines
  • Excellent verbal/written skills
  • Previous experience working with diverse populations, including economic diversity.
  • Possession a valid NYS Driver’s license and a driver's record considered acceptable by agency and insurance carrier.
  • Continuous use of a reliable, registered and insured vehicle.
  • Ability to work in the following software programs: Windows, Microsoft Office, and multiple online software platforms as required by funders and partner organizations
  • Ability to maintain confidentiality and handle confidential information with discretion
  • Ability to demonstrate behaviors and attitudes which support organizational mission, philosophy, and policies
  • Ability to work in a cooperative and helpful manner with all individuals
  • Willingness and ability to foster agency and program wide cooperation and teamwork through use of positive/constructive communication techniques
  • Provide effective, equitable, understandable and respectful quality care and services that are responsive to diverse cultural health beliefs and practices, preferred languages, health literacy and other communication needs.
