About the Role:
At Corus Home Health, we’re looking for a compassionate and licensed LPN to join our close-knit home health team in Albuquerque, NM. This is a field-based role offering autonomy, flexibility, and the chance to make meaningful, one-on-one connections with patients where they feel most comfortable — at home.

Key Responsibilities:

  • Deliver skilled nursing care under the supervision of a registered nurse (RN) and in accordance with patient care plans
  • Monitor patient conditions and report changes to the supervising clinician or physician
  • Perform treatments and administer medications as prescribed
  • Educate patients and caregivers on managing illness and treatment plans
  • Document all care activities accurately and promptly using EMR systems
  • Collaborate with interdisciplinary team members to support patient goals
  • Travel locally to provide care to patients throughout the Albuquerque area
  • Participate in team meetings, ongoing education, and quality improvement initiatives
Requirements:

What We’re Looking For:

  • Current LPN license in the state of New Mexico
  • Valid driver’s license, auto insurance, and reliable transportation
  • Experience in home health or a similar clinical setting (preferred but not required)
  • Strong communication and time management skills
  • Comfortable working independently in patient homes
  • Ability to adapt care to a variety of home environments and needs
  • Commitment to compassionate, high-quality patient care
  • Tech-savvy enough to document care using mobile EMR systems
