About the Role:
At Corus Home Health, we’re looking for a compassionate and licensed LPN to join our close-knit home health team in Albuquerque, NM. This is a field-based role offering autonomy, flexibility, and the chance to make meaningful, one-on-one connections with patients where they feel most comfortable — at home.
Key Responsibilities:
- Deliver skilled nursing care under the supervision of a registered nurse (RN) and in accordance with patient care plans
- Monitor patient conditions and report changes to the supervising clinician or physician
- Perform treatments and administer medications as prescribed
- Educate patients and caregivers on managing illness and treatment plans
- Document all care activities accurately and promptly using EMR systems
- Collaborate with interdisciplinary team members to support patient goals
- Travel locally to provide care to patients throughout the Albuquerque area
- Participate in team meetings, ongoing education, and quality improvement initiatives
What We’re Looking For:
- Current LPN license in the state of New Mexico
- Valid driver’s license, auto insurance, and reliable transportation
- Experience in home health or a similar clinical setting (preferred but not required)
- Strong communication and time management skills
- Comfortable working independently in patient homes
- Ability to adapt care to a variety of home environments and needs
- Commitment to compassionate, high-quality patient care
- Tech-savvy enough to document care using mobile EMR systems
