Manager - Case Management Social Work at LifeBridge Health – Baltimore, Maryland

Job Summary: Responsible for the oversight of daily operations, staffing, and workflows for Care Management Department. This includes supervision of social work staff. Provides clinical social work services to patients/residents and families, regarding the following common problems: adjustment to institutional living, financial concerns, family relationships, difficulty understanding/accepting medical/diagnoses/procedures/treatments, changes relating to aging, powerlessness, discharge planning and counseling related to death and dying.

QUALIFICATIONS: Master of Social Work; Current Maryland LCSW

5-7 years experience required
