Patient Care Coordinator at Valley County Health System – Ord, Nebraska

We’re Hiring! Join our team as a Patient Care Coordinator with a focus on Chronic Disease Management.

Are you passionate about helping patients take control of their health and improve their quality of life? We’re looking for a dedicated Patient Care Coordinator to join our Valley County Health System (VCHS) team. This is full-time position working 5 days per week.

In this role, you will:

• Provide education and support to patients living with chronic conditions such as diabetes, heart disease, COPD, and hypertension
• Develop individualized care plans
• Monitor patient progress and outcomes
• Collaborate with interdisciplinary healthcare teams
• Empower patients to build sustainable, healthy habits
• Provide direct patient care within the medical clinic

The ideal candidate is compassionate, organized, able to be a self-starter, can work independently, and is committed to patient-centered care. Experience in care coordination, patient education, and chronic condition management is preferred.

Applicants must have current Nebraska licensure as a Registered Nurse.
