Job Title: Patient Enrollment & Care Coordination Specialist

Position Summary:

Finney's Institutional Pharmacy is seeking a professional, organized, and compassionate Patient Enrollment & Care Coordination Specialist to join our pharmacy team. This individual will play a key role in onboarding new patients, coordinating enrollment activities, and guiding patients through the pharmacy workflow from start to finish. The ideal candidate is an excellent communicator who can multitask in a fast-paced environment while providing exceptional customer service.

Responsibilities:

  • Answer inbound phone calls and assist patients, providers, and caregivers with enrollment questions and requests
  • Make outbound calls to patients, physician offices, insurance companies, and referral sources as needed
  • Collect, review, and enter patient information accurately into internal systems
  • Coordinate and manage new patient enrollment workflows from intake through onboarding
  • Guide patients through pharmacy processes, explaining next steps clearly and professionally
  • Communicate with internal departments to ensure timely processing of referrals and prescriptions
  • Maintain organized and accurate patient records while ensuring confidentiality and HIPAA compliance
  • Follow up on missing documentation, authorizations, or required information
  • Provide excellent customer service and build positive relationships with patients and healthcare partners
  • Adapt to changing priorities and assist with additional administrative or coordination tasks as needed
  • Other duties as assigned

Qualified candidates will possess the following skills and experience:

  • Communication & Service: Strong verbal/written communication and professional phone etiquette focused on high-quality customer service.
  • Workflow & Tech Management: Highly organized, detail-oriented, and tech-savvy professional capable of multitasking, data entry, and adapting to fast-paced team or independent workflows.
  • Healthcare Industry (Preferred): Experience in a medical, pharmacy, or patient coordination environment with knowledge of HIPAA and confidentiality standards

Certification Requirement: National Certification - CPhT (Certified Pharmacy Technician)

Working Conditions:

  • Normal working hours: Monday through Friday, 9:00 AM to 6:00 PM, with additional time as required based on daily workloads
  • Saturday shifts may be required to support customers
  • Participation in the on-call rotation is voluntary, with incentive pay provided
  • Data entry team members must work well together as a team

Physical Requirements:

  • Sitting and working at a computer for up to 8 hours a day, minus scheduled breaks
  • Operating a desktop keyboard and mouse
  • Using a desk phone with either handset or wireless headset
  • Communicating with employees, patients, caregivers, and physicians by phone, fax, and email in a courteous and professional manner
