Licensed Clinical Social Worker - Full Time in Blountstown, Florida at Calhoun Liberty Hospital Association, Inc

A Licensed Clinical Social Worker will serve as the vital connecting piece within the Calhoun-Liberty Hospital and EMS CORE Network, ensuring seamless warm handoffs from EMS and hospital staff to the receiving clinic. This position is essential to maintaining continuity of care and preventing individuals seeking opioid recovery from falling through service gaps.

Serve as the vital connecting piece within the Calhoun-Liberty Hospital and EMS CORE Network, ensuring seamless warm handoffs from EMS and hospital staff to the receiving clinic. This position is essential to maintaining continuity of care and preventing individuals seeking opioid recovery from falling through service gaps.

Serve as Patient Navigator for CLH EMS Paramedicine program and CLH Rural Health Clinic with focus on Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) programs. Managing each patient's path after first contact: scheduling and onboarding visits, coordinating the appointments a referral requires, following up on missed visits, and removing barriers such as transportation or paperwork. Makes no clinical decisions and escalates to the Continuum of Care Coordinator when one is required.

Assists CLH Case Manager/Swingbed Coordinator with overcoming barriers to patient placement and obtaining community resources to provide a safe discharge.
Qualifications:
Master's Degree in Social Work from a CSWE accredited institution
2 years of experience as a Social Worker in a rural environment preferred
Current and unrestricted DOH-issued LCSW
Ability to read, speak, and write the English language
