Job Description
Title: Housing Programs Case Manager

Supervised By: Housing Program Manager

Home Start, Inc. operates programs designed to prevent homelessness, interrupt the cycle of homelessness, and promote self-sufficiency. Our programs are designed to ensure homelessness in San Diego is rare, brief, and non-recurring.
Job Summary:
Provide trauma-informed case management and supportive services to program participants in Home Start housing programs through home visits, office-based support, and community engagement. Support participants in achieving housing stability and self-sufficiency through individualized planning, resource coordination, and ongoing support.
Key Responsibilities
Case Management Support
• Conduct comprehensive intakes, assessments, and individualized case planning.
• Develop and monitor individualized service plans for assigned caseload to support housing stability, education, employment, parenting, life skills, and overall well-being.
• Provide strengths-based coaching, crisis intervention, and problem-solving support.
• Connect participants to internal and community-based resources and support services.
• Support development of independent living, parenting, and tenancy skills.
• Adhere to Home Start principles and practices, including trauma-informed care.
• Assist participants in navigating social service, child welfare, and justice systems where applicable.
• Utilize trauma-informed de-escalation techniques to support participant safety and engage emergency services when necessary.
Collaborative Coordination
• Coordinate services with partner agencies to ensure wraparound support.
• Participate in case conferencing, cross-agency meetings, and collaborative planning.
Documentation, Reporting, and Compliance
• Maintain accurate, timely, and complete case records and housing documentation in required databases, such as Clarity, HMIS, Sage, etc.
• Assist with drafting and submitting reports.
• Ensure confidentiality and ethical handling of participant information.
Outreach and Engagement
• Conduct outreach to eligible potential participants and referral partners.
• Engage participants using trauma-informed and culturally responsive practices.
Administrative and Team Participation
• Participate in meetings with supervisor and team, trainings, and collaborative convenings.
• Maintain flexibility for field-based work and occasional evening hours.
• Escalate complex cases or safety concerns to your supervisor.
• Perform other duties as assigned.
Qualifications:
Education
Bachelor’s degree in Social Work, Human Services, Psychology, or a related field preferred. Relevant experience may substitute for education.
Experience
• Experience in housing navigation, case management, or other services for populations served including transition-age youth, foster youth, survivors of domestic violence, pregnant or parenting youth, unhoused, or at-risk of homelessness required.
• Experience working with pregnant or parenting youth preferred.
• Experience collaborating with landlords, housing programs, or rental assistance initiatives preferred.
Knowledge, Skills, and Abilities
• Knowledge of housing resources, landlord-tenant practices, and community service systems in San Diego County.
• Ability to engage population served with complex needs using trauma-informed and culturally responsive approaches.
• Strong organizational, documentation, and time-management skills.
• Ability to work independently in community settings and collaboratively within a multidisciplinary team.
• Demonstrated commitment to culturally responsive, inclusive, and collaborative service delivery.
• Knowledge of community resources.
• Strong oral and written communication and computer skills.
Preferred Language Capacity
Bilingual in English and Spanish or another language reflective of the populations served preferred.
Additional Requirements
• Valid California driver’s license, acceptable driving record, current automobile insurance, and reliable transportation.
• Ability to travel throughout San Diego County.
• Must pass a criminal background check.
Physical Requirements
• Ability to sit for extended periods while completing documentation and attending meetings.
• Frequent travel by car to meet with participants and attend agency or community meetings.
• Ability to perform essential job functions with or without reasonable accommodation.
Work Environment
• Combination of field-based work and shared office environment.
• Use of standard office equipment.
• Noise levels vary from quiet to moderate.
