Responsibilities
The Patient Care Navigation Specialist provides peer recovery support, coaching, and care navigation throughout the recovery continuum, from MSU admission through discharge. Utilizing lived experience, recovery support principles, continuity focused engagement, and evidence-based coaching practices. This role helps participants navigate transitions in care, overcome barriers to recovery, connect with resources, achieve recovery goals, and build a foundation for long-term recovery success. The role is designed to complement clinical, medical, and care coordination functions by providing a steady supportive presence. The specialist collaborates with participants, treatment teams, families, and community partners to promote engagement, continuity of care, and positive recovery outcomes.
- Provide Coaching, Navigation, and Recovery Support to Participants in Accordance with Hazelden Betty Ford Standards of Care
- Facilitate Recovery Transitions and Continuity
- Support Program Operations, Documentation, Compliance, and Team Collaboration
Qualifications
Required Qualifications:
- High School Diploma or the equivalent
- Peer Recovery Certification or obtain within 90 days of hire
- Experience providing peer support in a 12-Step program as well as other recovery pathways
- Training/proficiency in computer applications, and Microsoft products, Office, Word, PowerPoint, Windows, and Excel, along with the use of internet as work tools
- Public speaking, group presentation experience
- Must be age 18 or older
- Must have a current driver’s license
- Current CPR certification or obtain within 30 days
- Certification must be from American Red Cross or American Heart Association.
Preferred Qualifications:
- Bachelor’s Degree preferred, but not required if peer certification and relevant experience are strong
- Prior experience with withdrawal management, residential treatment, recovery management, care navigation, case management, or continuing care.
- Experience supporting patients through transitions in levels of care
- Knowledge of community recovery resources, mutual support groups, and recovery pathways
