Job Description

Job Summary

The Contract Services Coordinator LPN is responsible for coordinating and monitoring visit utilization with contract agencies, including reviewing frequencies, scheduling, and plotting contract visits. This role ensures that contracted services are delivered in alignment with patient plans of care, agency policies, and regulatory standards. The LVN serves as a liaison between contract agencies, interdisciplinary teams, and management, providing timely communication and support to optimize patient care and ensure proper service utilization.

Job Qualifications

Education: Graduate of an accredited school of vocational/practical nursing.


Licensure: Active state LVN/LPN license (or multi-state NLC license). Current driver’s license required.


Experience: At least 1 year of experience as a licensed vocational/practical nurse in home health preferred.


Skills:

  • Knowledge of home health nursing standards within LVN scope of practice.

  • Strong organizational and communication skills.

  • Computer proficiency with EMR systems, Excel, or Google Sheets.

  • Ability to work independently, prioritize tasks, and problem-solve under deadlines.


Transportation: Reliable transportation and valid/current auto liability insurance.


Environmental / Working Conditions

  • Routine office environment, standard hours (8:00 am–5:00 pm).

  • Noise level may be moderately high.

  • Flexibility for extended hours when needed.

  • Local travel may be required with exposure to inclement weather.

  • Possible exposure to common office materials such as toner or cleaning supplies.


Physical and Mental Effort

  • Prolonged sitting with some standing required.

  • Ability to work under deadlines and stressful conditions at times.

  • Manual dexterity and hand-eye coordination for computer and clerical tasks.

  • Requires clear verbal and written communication, strong interpersonal skills, and critical thinking to resolve issues


Essential Functions

Evaluation

  • Communicate visit utilization recommendations to contract agencies and interdisciplinary teams in a timely manner.


  • Review and monitor patient frequencies to ensure accuracy and appropriateness of service delivery.


  • Participate in coordinating care with management and interdisciplinary teams to support patient needs.


  • Maintain effective, professional communication with contract agencies and respond promptly to inquiries.


  • Collaborate with clinical staff and management to address discrepancies in visit frequencies or scheduling.


  • Ensure timely scheduling and plotting of contract visits to align with the plan of care.


  • Demonstrate professional growth, competency, and adherence to agency standards.


  • Promote agency philosophy, values, and administrative policies to ensure consistent, high-quality care.
