Job Description
POSITION SUMMARY
The Home Care Manager supervises CHHA staff in their daily activities to ensure the delivery of care, ensuring compliance with regulatory COPs and State guidelines; this includes but not limited to field visits as well as on-site locations. This position assists in developing policies and procedures as they relate to the CHHAs. The Home Care Manager assures continuing compliance with all license and certification requirements. This position performs administrative duties to include; coordinating and orienting new employees, troubleshooting problem areas, and managing the CHHA team. This position requires, resolving staff issues and disciplinary actions, participating in performance reviews, monitoring budgetary practices, preparing staff schedules, reconciling pay-roll, developing and implementing performance improvement activities, and providing off-hours administrative and clinical support. This position also collaborates with the Director of Clinical Services to maintain regulatory standards as well as quality standards. This roll will participate in QAPI meetings, audits and reviews to ensure quality standards are met. The Home Care Manager serves as a key communicator to the CHHA team for organizational updates, policy changes and education needs. This position establishes and maintains communication with IDT and other professionals to promote mutual understanding of problems and plans of care. This position supports organizational goals by providing quality customer service, participating in performance improvement efforts and demonstrating a commitment to teamwork and cooperation.
QUALIFICATIONS
EDUCATION: Graduate of an accredited required. Bachelors in nursing or Allied Health preferred.
LICENSE/CERTIFICATION: Current licensure as a Registered Nurse in the State of NJ. BLS required. National Certification in specialty preferred. Drivers License with proof of registration and insurance required.
EXPERIENCE: 2-5 five years clinical experience required with the frail or elderly population. 2-5 years of supervisory experience preferred. Excellent verbal and written communication skills as well as leadership skills required. Proficiency in Clinical Applications preferred at time of hire; incumbents within position will be trained appropriately and then skill will be required for this position within 30-60 days from date of hire.
PERFORMANCE EXPECTATIONS Demonstrates the ability to perform the department specific competencies as listed on the Assessment and Evaluation Tool. ENVIRONMENT Potential for exposure to the hazards and risk of the hospital environment, including exposure to infectious disease, hazardous substances, blood-borne pathogens, and potential injury Physical demands include but are not limited to consideration stooping, kneeling, crouching, pushing heavy equipment and excessive standing and walking. Lifting approximately 25-50 lbs. frequently; lifting 50-10 lbs. with assistance occasionally. Some departments require the ability to distinguish colors. Essential functions for this position are listed on the Assessment and Evaluation Tool.
REPORTING RELATIONSHIP This position reports to department leadership and supervises incumbents as assigned.
